Position Summary:
Reporting to the Manager of the Wellness Program, the Social Worker II will, as a member of the Wellness interdisciplinary team, work collaboratively to address the psycho-social needs of our clients. Through interventions in the areas of assessment, referral, psycho-social intervention, group facilitation, education and discharge planning, the Social Worker collaborates with the client and community providers to support the best possible client outcomes. The Social Worker is responsible for the delivery of patient and family centered service that is consistent with the vision and values of the hospital and the standards of professional social work practice. The Social Worker complies with the Occupational Health and Safety standards for the hospital and is proactive in promoting a safe work environment.
Qualifications:
Experience in Adult Mental Health – Required
Experience in applying CBT and DBT skills to adults with mental illness - Required
Candidate must demonstrate a wide range of clinical and organizational skills to address the discharge-related needs of clients and their families within the Health Care System
Knowledge and skills in a range of therapeutic approaches, including brief therapies, motivational interviewing and general understanding of CBT and the foundations of DBT with individuals, and groups
Experience in assessment and in the development of a comprehensive intervention plan based on identified needs
Thorough knowledge of Mental Health support services and resources in Windsor and Essex County available to clients
Excellent interpersonal and communication skills
Committed to the values of the organization and courtesy in patient care
French language proficiency is considered an asset
Valid Ontario Driver’s License - Required
Education and/or Certification:
Masters of Social Work (MSW) - Required
Current Certification of Registration issued through the Ontario College of Social Workers & Social Service Workers – Required
DBT training – Required
CBT training – Required
