Position Summary
We’re hiring a Licensed Practical Nurse (LPN) for a hybrid role supporting Satori’s Collaborative Health Program. You’ll partner directly with an in-clinic physician, work closely with patients, and play a key role in proactive, patient-centered care.
What You'll Do

Educate and empower patients to take an active role in their health.

Collaborate with a physician and clinic team to support optimal care outcomes.

Conduct health assessments and track patient progress.
-
Support treatment planning, preventative screenings, and health goal setting.

Balance time between assigned clinic(s), your home office, and occasional visits to our Calgary head office.
What We're Looking For
-
Current LPN license in Alberta, in good standing.

Minimum 3–5 years of LPN experience, preferably with a variety of patient populations or in a primary care setting.

Excellent communication and interpersonal skills.

Strong interest in preventative health and chronic disease management.

Comfortable working independently in a hybrid setting and collaborating within a team.
