About the role

Job Summary:
In accordance with the Mission, Vision and Values, and strategic directions of Provincial Health Services Authority patient safety is a priority and a responsibility shared by everyone at PHSA, and as such, the requirement to continuously improve quality and safety is inherent in all aspects of this position.

Under the direction of Oncology/Hematology/BMT social workers and reporting to the Professional Practice Leader of Social Work, the Family Resource Coordinator is primarily responsible for providing initial assessments and assisting patients and families in the Oncology/Hematology/BMT program with practical resources and tangible needs.

Duties/Accountabilities:

  1. Assess patient and family need for practical supports and assist families in accessing and securing resources internal and external to the hospital.

  2. Assess family coping skills and prepare written assessments and reports to refer to social work when indicated, including child protection concerns.

  3. Assess families’ needs and provide support services such as transportation, parking, and meal vouchers. Assist families in accessing and completing appropriate forms and applications.

  4. Provide basic counselling to families regarding the practical stressors of chronic illness and ongoing medical treatment.

  5. Liaises and works with social work staff to coordinate and plan responses to support financial and practical needs of patients and families.

  6. Assist the program social work staff with managing and communicating information on community resources, and creating systems to update and communicate this information to families.

  7. Performs other related duties as assigned.

Qualifications:
Qualifications

Education, Training and Experience

  • A level of education, training, and experience equivalent to a Bachelor’s degree in an allied health discipline
  • Minimum of one (1) year experience in a health care setting working with a wide range of service providers.

Skills and Abilities

  • Knowledge and experience in family support and pediatrics.
  • Extensive knowledge of community agencies and resources.
  • Ability to liaise effectively with staff at all levels of the organization and with partners in other health centers and community agencies.
  • Excellent interpersonal, oral and written communication skills including training in reflective listening and validation.
  • Ability to work independently with limited supervision.
  • Self-directed and highly motivated with excellent organizational skills.
  • Demonstrated sensitivity, confidentiality, and commitment to family-centered care.
  • Ability to collaborate effectively within a multidisciplinary care team.
  • Ability to operate related equipment including word processing, spreadsheet and database programs at an intermediate level.
