Case Management Outreach Worker
About the role
Aniskomohcikan (Connect/Connection)(Un-scoo-moo-chi-gun) is a program offered by the Grande Prairie Friendship Centre to address the diverse and complex needs of Indigenous Elders in the Grande Prairie community. The program aims to ensure Indigenous Elders have access to safe housing, nutritious meals, healthcare, transportation, and social supports.
Hours: 28/hrs per week (schedule set based on Elders needs)
Qualifications:
- Human Services diploma or degree
- Knowledge of the Indigenous culture
- Experience working with mental illness and substance abuse
- Knowledge of community resources and supports
- Previous outreach & community work experience
- Experience working and interacting effectively and respectfully with participants in their home and in the community
- Experience in guiding and supporting participants to maintain healthy home environments, sustainable housing, nutrition and other supports.
- Experience working with individuals with behavioral challenges
- Excellent oral and written communication skills
- Ability to meet deadlines and work under pressure
- Valid Class 5 Driver’s License with reliable vehicle
- Ability to hold $2 million liability insurance
- Cleared Criminal Record and Child Intervention Record Check
- Valid First Aid
- Mental Health First Aid
- ASIST
- Strong knowledge of Grande Prairie and surrounding communities an asset
Duties and Responsibilities include but are not limited to:
- To report verbally daily to the Program Coordinator
- To immediately report and discuss any concerns that might arise to the Program Coordinator
- Provide ongoing support to participants in navigate housing, healthcare, social supports and other essential services.
- In-home supports such as light housekeeping, meal preparation, transportation to appointments and social supports
- Connect participants to community and social programs offered by GPFC and other agencies.
- Refer participants in crisis to appropriate resources
- Ensure participant files are maintained and updated
- Provide information and referrals to appropriate agencies
- Treat all participants with dignity and respect
- Foster and maintain excellent relationships with professionals involving participant’s needs
- Maintain confidentiality at all times
- Maintain files, case notes and daily, weekly and monthly stats
- Maintain positive working relationships within the GPFC
- Follow policy and procedure guidelines of the GPFC at all times
