About the role
The Resident and Family Community Manager is a key leadership role in our long-term care
facility, responsible for fostering a supportive and collaborative environment for residents and
their families. This position provides direct oversight of the admissions and discharge process,
ensures accurate and timely completion of RAI-MDS assessments via InterRAI LTCF, supervises
the RAI-trained RPNs, and acts as the primary liaison between residents, families, and the care
team. The Resident and Family Community Manager also chairs care conferences and
interdisciplinary meetings to ensure high standards of person-centred care.
Hours: Monday-Friday
Status: Full-Time (new vacancy)
Pay Rate: $112,558-$132,311
Qualifications:
Education:
Regulated healthcare professional in good standing with the applicable Regulatory body
Certification or demonstrated experience with InterRAI LTCF and RAI-MDS coding
Experience:
Minimum of 3 years experience in a long-term care or community health setting
Previous supervisory or leadership experience preferred
Required Skills:
Strong understanding of the Long-Term Care Homes Act, 2007 and its regulations
Excellent communication, interpersonal, and conflict resolution skills
Ability to manage multiple priorities and thrive in a dynamic care environment
Proficiency with electronic health records (EHRs), including PointClickCare or
equivalent
