The Toronto Grace Health Centre (TGHC) provides, specialized care to those individuals who require Complex Continuing Care, Post-Acute Care Rehabilitation, Transitional care, and Palliative Care services. The care and services provided at TGHC help facilitate the patient flow through the health care system and ensures recovery to those individuals who may return to their homes or to Long Term Care (LTC) facilities in their communities. TGHC operates in 2 sites – the Main site (Church& Bloor St) and our Harbour Light site and Specialized Care Centre (Victoria Park & Mc Nicoll). POSITION SUMMARY:
We currently have an exciting opportunity available for a Recreation Therapist to join the team at our Harbour Lights Scarborough location. The Recreation Therapist practices in accordance with the standards of practice of Therapeutic Recreation Ontario, the Canadian Therapeutic Recreation Association and in accordance with the mission and philosophy of the Salvation Army Toronto Grace Health Centre. The Recreation Therapist is responsible for the assessment, planning, implementation, and evaluation of Therapeutic Recreation (TR) interventions and/or services for assigned patient care unit(s) and/or programs. The aim of the service is to assist the patient to an optimal level of health and independence in recreation and leisure pursuits and to improve quality of life. When no active therapy has been planned, the Recreation Therapist will participate in the planning of care as needed. The role entails active involvement with the patient/family and other members of the interdisciplinary team to determine the most appropriate and effective overall plan of care for the patient. The Recreation Therapist demonstrates the knowledge and judgement to provide programming for patients from diverse patient populations including residents living with complex mental health illness & elderly patients with frailty and dementia in a transitional care setting. MAIN RESPONSIBILTIES:
- Assumes responsibility and accountability for the provision of patient care utilizing current therapeutic recreation theory, best practices, and the principles of patient centered care, in accordance with TRO standards of practice.
- Complies with regulations, values, ethics, and standards as specified by TRO.
- Identifies patient/family concerns, needs, wishes, and goals.
- Completes and documents a therapeutic recreation admission assessment for residents living with complex mental health illness & elderly patients with frailty and dementia in a transitional care setting as per the TR clinical documentation protocol.
- Contributes to the development of the individualized plan of care based on patient/family concerns, needs, goals and therapeutic recreation assessment.
- Monitors and documents patient’s progress in TR interventions.
- Maintain an accurate patient attendance record for groups and individual programs.
- Contributes to the evaluation of the patient’s plan of care in collaboration with the patient/family and members of the interdisciplinary team.
- Actively participates in patient/family, team care conferences as appropriate.
- Collaborates with other caregivers to incorporate therapeutic functional activities into delivery of patient care.
- Develops recreation programs to meet patients’ therapeutic recreation needs.
- Applies critical thinking and leadership skills to enhance problem solving and decision making.
- Demonstrates conflict management skills and seeks to resolve situations requiring resolution.
- Participates in staffing coverage plan.
- Prepares and distributes the therapeutic recreation monthly activity calendar.
- Serves as liaison with outside agencies connected with Recreation Programs.
- Develops and maintains a community resource list for special programs and services.
- Maintains ongoing communication with Volunteer Services regarding the participation of volunteers in Therapeutic Recreation programs.
- Identifies budget requirements for special events and other therapeutic recreational activities.
- Communicates with colleagues, community agencies and vendors regarding information and services relevant to programs and patient services.
- Participates in both hospital-wide and unit-based QIP projects that improve the provision of care and service to patients and families.
- Maintains workload statistical data as required.
- Provides training, supervision, and assistance to volunteers.
- Performs other duties as required.
QUALIFICATIONS & EDUCATION:
- University or Post graduate College degree in Therapeutic Recreation.
- Eligible for membership in Therapeutic Recreation Ontario (TRO) and/or Canadian Therapeutic Recreation Association (CTRA).
- Proven ability to function effectively as a team member.
- Excellent interpersonal and organizational skills.
- Exceptional attendance record.
- Mental health experience, hospital-setting and with diverse communities preferred.
- Previous clinical experience with a special population.
- Communicates effectively in oral and written English.
- Working knowledge of applicable computer software.
- Current Basic Cardiopulmonary Life Saving course preferred.
- Must have physical strength, flexibility and endurance required to perform the role as deteremined by the physical needs of the position based on the assessment conducted by Occupational Health Services.
- Excellent verbal and written communication skills.
- Excellent attendance record.
