The Garry Hurvitz Center for Community Mental Health at SickKids (GH-CCMH) is a child and youth mental health center integrated with the Hospital for Sick Children (SickKids). GH-CCMH is also a member of the Thriving Minds Mental Health Access Collaborative with CAMH and SickKids and a core service provider in the Toronto sector of infant, child and youth mental health services funded by the Ministry of Health. GH-CCMH provides prevention, early intervention and health promotion services across the city, funded by MOH and other sources. As such, the Center has an ambitious agenda to provide the highest quality, evidence-based care, offering a range of services from prevention and early intervention to counseling and therapy, assessment and intensive supports. Services are offered in person and in virtual format. We are currently seeking applicants for the full-time regular position of a Case Manager. This position is dedicated to the transitions of children, youth, and their families between and among hospital and community based mental health services and supports including SickKids, CAMH, and GH-CCMH, a partnership known as Thriving Minds. As such, the role will support service navigation activities and case management for all three organizations with structured secondments to CAMH and SickKids. The goal of Thriving Minds is to work towards service coordination such that children, youth, families, and caregivers seamlessly experience the child and youth mental health system across the involved organizations. The position will work collaboratively with relevant system partners to support the clinical transition pathways. This is a clinical service position and will provide psychosocial assessments, intervention planning, referral support, case coordination, and brief supportive counselling to children and youth experiencing mental health concerns, and their families, to ensure they move in through and out of publicly funded mental health services in a timely and integrated manner.
KEY RESPONSIBILITIES
- Undertake psychosocial assessments to identify child/youth and family strengths, problem-solving abilities, interactions, attitudes, feelings and cultural factors in relation to the child/youth’s health and mental health, and the supports/resources available to the family.
- Develop and implement a plan of intervention that addresses coping, adaptation, adherence and quality of life through individual counseling, family work, group work, additional evidence-informed interventions, inter-professional consultation, case management, and community outreach and advocacy.
- Provide developmentally informed transitional support for children, youth and families referred among GH-CCMH, SickKids and CAMH.
- Provide family education, counselling and support through brief interventions.
- Facilitate multiple care aspects (e.g. case coordination, information sharing, implementation of practical clinical strategies, system navigation, caregiver support).
- Coordinate and provide care that is safe, timely, effective, efficient, equitable, patient and family centered and meaningful to children, youth and families.
- Develop effective working relationships with relevant hospital and community based mental health service system partners to collaborate/communicate throughout the entire case management process.
- Practice in a manner that is consistent with the philosophy of continuous quality improvement and evidence-based practice.
- Prepare client documents in an Electronic Health Record and submit reporting information according to established procedures and deadlines.
- Maintain up-to-date knowledge of therapeutic services and best practices within the industry.

- Provides support in developing the Thriving Minds Coordinated Access Model.
- Complies with the organization’s vision, policies and procedures.
- Performs other tasks as assigned by management.
Qualifications
- Master of Social Work or equivalent in a mental health discipline and a minimum of three years of related clinical experience or equivalent combination of education and experience.
- Membership in good standing with relevant professional College.
- An understanding and demonstrated competencies in the bio-psycho-social model of assessment, developmentally informed care models, and key mental health diagnoses.
- Well-developed assessment and intervention skills, as well as skills in crisis management.
- Demonstrated skills in therapeutic engagement.
- Well-developed strengths in advocacy.
- Ability to collaborate effectively as a member of a care team and to take initiative and leadership when appropriate.
- Commitment to remain abreast of current literature and relevant developments in clinical practice.
- Excellent verbal and written communication skills to provide therapeutic services to clients and families.
- Excellent problem-solving skills to analyze complex client issues and recommend solutions.
- First Aid/CPR is an asset.
- Good knowledge of MS Office Suites (Outlook, Word, Excel, PowerPoint).
- Ability to incorporate values of a culturally competent practice of diversity and inclusiveness in all aspects of work.
- Ability to work flexible hours, including evenings and weekends.
