Title: Manager, Chronic Disease and Serious Illness

Salary: $112,090 – $131,870 – $151,650 per year (pay range explained below)

Contract: Permanent

Hours: 37.5 hours, Monday – Friday (core hours 9am – 3pm)

Location: Park Royal, West Vancouver – Hybrid (50% in office) & Remote work available

About The Role As Manager, Chronic Disease and Serious Illness, you will – provide leadership to a team of clinicians with specialty knowledge and expertise in chronic disease, serious illness, palliative and end of life care. Additionally, to support care in these areas of speciality practice, you will lead provincial consultation, practice support and education for the home and community care programs, palliative and end of life care in First Nations communities.

You are also responsible for leading the collaborative strategy development and support to FNHA Regional teams and First Nations Communities for clinical services for clients with chronic disease, serious illness and who are receiving home and community care services and/or palliative or end of life care. You will work in partnership with regional and provincial partners, you will lead with a focus on culturally safe and innovative inter-professional team-based quality care and you will support the ongoing design, planning, implementation, and evaluation of services that enable innovative, evidenced based, best practice care.

About You To be successful in this role you will have/be –

  • A master’s degree in a relevant field, or an equivalent combination of education, training and experience
  • Relevant professional development in areas of specialty clinical practice, leadership, process improvement, change management.
  • Recent, relevant experience, of at least 8 years, at a senior level working within a complex healthcare setting with a focus on Chronic Disease, Serious Illness, Home and Community Care, Palliative and End of Life Care.
  • Demonstrated knowledge and ability in leadership, strategic and systems level planning, clinical health care practice, program planning, development, implementation and evaluation.

The Pay Range is the minimum and maximum annual salary based on full time equivalent hours.

Incumbents are typically hired, transferred, or promoted between the minimum and midpoint of the range based on their knowledge, skills, abilities and experience in relation to the role requirements.

The top 10% of the pay range is for the incumbents who are industry experts in the job with the combination of exceptional experience and competencies needed to perform all duties and responsibilities at a superior capability level. Internal employee compensation will be determined as per the FNHA Employee Total Compensation Policy.
