Your Opportunity:
The Transition Services (TS) - Community Care Coordinator RN screens, assesses, coordinates, and evaluates the needs of patients and families who cannot function independently in the community and now requirement placement.
The Transition Services Coordinator (TSC) must have a working knowledge of community supportive living and LTC sites and the resources available to facilitate placements. The TSC collaborates, negotiates and works with families/clients, government programs (AISH, OPT, OPG), internal partners (IHC, ISFL) and external partners (funded owner operated facilities) to coordinate alternative placements. The TSC plays a critical role supporting families and clients navigate the Continuing Care healthcare system. Transition Services is looking for a casual employee with up to full time availability to learn the TS Community Coordinator role at Southport Atrium. This casual role can be offered up to 4 days/week with travel to clients in the community, SL and LTC.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS.
PLEASE NOTE: Provisional Permit Holders shall be paid $39.58 /hour - $54.16/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable.

Required Qualifications:
Completion of an accredited nursing education program.
Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA).
Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
If you have not practiced as a Registered Nurse within the last five (5) years, completion of the following free courses is required: CRNA Medication Management - Chapters 1 - 4 (https://connect.nurses.ab.ca/home/%2Flearning-and-development%2Flearning-modules%2F) and the NextGenU Health Assessment Resources Certification (https://courses.nextgenu.org/course/view.php?id=271#section-0)
Additional Required Qualifications:
Minimum of 3 years of experience in community-based services such as Transition Services, Integrated Home Care (IHC), or Integrated Supportive & Facility Living (ISFL). Strong assessment and critical thinking skills, along with excellent interpersonal, communication, conflict management, and organizational abilities. Must be able to work effectively within a multidisciplinary team, including managers, allied health professionals, physicians, and nursing staff. Must be comfortable managing an unpredictable caseload and adapting to frequent interruptions and multitasking. Flexibility is essential to respond to healthcare system needs and urgent client or family situations (e.g., emergency placements). Must be physically capable of walking a minimum of 3 km per shift, including stairs, while carrying resource materials weighing up to 25 lbs. Ability to perform up to 6 hours of keyboarding per shift (typing speed of up to 50 words per minute) and complete written documentation while using various software programs. Proficiency with computer systems such as NetCare, SCM, Strata Pathways, and Connect Care is required.
Preferred Qualifications:
Baccalaureate Degree in Nursing preferred; however, skills, and experience relevant to nursing practice in community sector will be considered. High level of independence and decision-making ability
