Edmonton Southside Primary Care Network – Primary Care Registered Nurse
The Edmonton Southside Primary Care Network was the first PCN in Alberta. Over the past 17 years, the ESPCN has grown to become the largest PCN in Edmonton and consists of a team of health care professionals who are passionate about creating healthier communities. We are currently seeking an experienced Primary Care Registered Nurse to join our team!
Position Overview:
The Primary Care Registered Nurse (PCRN) will provide comprehensive nursing services and care to patients within the Primary Care Network. The PCRN will assess, plan, implement and evaluate nursing care plans to patients of all ages. The PCRN will work to their full scope of practice, including performing restricted activities as outlined in the Health Professions Act in the primary care setting.
The Primary Care Registered Nurse (PCRN) will be an integrated team member at the ESPCN Central Office. The PCNRN will engage in a wide range of activities to support comprehensive patient care, including consultation and interventions for patients referred to the Central Office, program development, review, and delivery (in-person and virtual), nursing support for centrally delivered programs and services, and care coordination to manage patients referred to the central office. The Primary Care Registered Nurse – Central Office will also provide occasional PCRN coverage in ESPCN clinics and engage physicians who do not have a co-located PCRN to establish a PCRN role within their clinic.
The Primary Care Registered Nurse will work with the family physicians in the community, other health professionals and local and regional programming in a multidisciplinary environment to ensure a high quality of evidence-based health care to the practice population
Key Roles & Responsibilities:
· To work collaboratively in a centered patient-centered, integrated team-based practice within a primary care setting.
· To participate in program development, review, and evaluation.
· To engage in care coordination activities and proactive care and problem-solving.
· To participate in the culturally appropriate care of individual patients, practice populations and associated communities.
· To provide comprehensive nursing services and care to patients at the Central Office and in the family practice setting.
· The PCRN will assess, plan, implement and evaluate care to patients of all ages and work with patients with complex healthcare needs.
· To make professional, competent, and autonomous decisions with patients as a member of the multidisciplinary team.
· To deliver and facilitate group programs and visits.
· To act as a nursing resource to other multidisciplinary team members, healthcare institutions and community agencies or programs.
· To ensure accurate, objective, and timely clinical documentation in the appropriate medical record.
Competencies & Behaviours
· Baccalaureate Degree in Nursing
· Current licensure with College & Association of Registered Nurses of Alberta (CRNA) licensure
· Current Basic Life Support (BLS) for Healthcare provider
· Vehicle/license required
· Previous experience:
o Primary care setting with a wide variety of patient populations
o Electronic Medical Record
o Group facilitation/instruction
o Health behaviour change methods (eg Motivational Interviewing)
o Program development and evaluation
o Care coordination and case management
· Understand the Alberta privacy laws and act according to those laws
· Should behave responsibly and demonstrate accountability
· Excellent written and verbal skills
· Evidence of problem solving and critical thinking skills
· Flexible and adaptable
· Takes initiative; self starter
· Ability to work in ambiguous and ever-changing situations
