Nursing Homes Without Walls

We are currently seeking a Wellness Coordinator to join our team at Nursing Home Without Walls (NHWW). The purpose of the position is to assist with the planning, organizing, development, and implementation of social engagement opportunities for the NHWW clients in the community. The Wellness Coordinator will commit to meeting the individual needs of the clients who utilize NHWW for services using a client-centered care approach. This position is for a one-year contract with the possibility of an extension.

Responsibilities

  • Design and activate wellness programs that builds community, drives engagement, and improves client’s social engagement.
  • Assist in the planning and implementation of a therapeutic recreation program.
  • Conduct one-on-one in-home visits.
  • Motivate and inspire clients to achieve their full potential by engaging them in programs and activities of their interest and choice.
  • Provide educational opportunities for clients.
  • Demonstrate respect and empathy for all clients and team members.
  • Assist in developing programs that address social isolation and loneliness for clients and their caregivers.
  • Assist in the creation of a calendar of events that will be distributed to clients.
  • Assist clients during in-home ‘wellness visits with activities of daily living (ADL’s).
  • Foster and maintain community-linked partnerships.

Requirements/Qualifications

  • University or College education in Gerontology, Therapeutic Recreation, or post-secondary completion of certified course in Recreational Programming. Education or experience in gerontology and working with the aging population would be considered an asset.
  • Current Certification in Cardio-Pulmonary Resuscitation.
  • Good knowledge of aging, age-friendly communities, and resources.
  • Strong organizational and multi-tasking abilities.
  • Ability to initiate, design, implement/ facilitate social engagement activities and educational opportunities for clients, which includes both group and one on one sessions.
  • Comfortable conducting one-on-one visits in clients home.
  • Excellent computer skills and experience with Microsoft Word, Excel, PowerPoint and Canva.
  • Self-motivated and able to work independently with minimal supervision.
  • Excellent communication skills both written and verbal and the ability to build positive relations and collaborations.
  • A valid driver’s license and access to a vehicle to do home visits is required.
  • Good previous work and attendance record.
  • Must provide a Criminal Record Check.
  • Must pass Vulnerable Sector Social Development Check.
  • Must be physically able to perform assigned duties and capable of demonstrating the ability to carry out the key activities outlined above.
