About the role

Job Description

What you’ll be doing

  • Responsible for managing the co-ordination and completion of the Resident Assessment, Resident Assessment Protocols (RAPs) and care plans as assigned.
  • Complies with MOH regulations regarding completion of the RAI- MDS process.
  • Completes accurate coding of the RAI - MDS and care plan documentation for all residents.
  • Continually updating knowledge base related to data entry and computer technology.
  • Chair RAI meetings as required.
  • Any other duties as assigned

Qualifications

  • 3-5 years’ experience in Long Term Care, experience with MDS.
  • Current registration as RPN in good standing with the College of Nurses is required.
  • Current RAI-MDS certification and AIS certification is required.
  • Must be able to maintain complete confidentiality of information.
  • Knowledge and experience with adult education strategies/facilitation.
  • Strong analytical skills; knowledge in trending and analyzing data.
  • Strong understanding of Point Click Care and RAI-MDS.
  • Must be flexible to work variable shifts, self-directed and able to function in a fast-paced environment.
  • Bilingual
  • Competent completing care plans from scratch
  • Passing AIS testing
