Community Health Centre Family Physician – Luther Court (Victoria PCN) (100-1873)
Position
The Luther Court Society is seeking a full or part time Family Physician (FP). The FP's primary role is to assess and diagnose acute and chronic disease/illness and provide treatment and follow-up. Related responsibilities include to prescribe and refill medications, order and interpret diagnostic tests, perform minor surgical procedures, provide management of chronic disease and end of life care, and refer to specialists and specialty programs as needed. The patient panel for a full time (1680 hours a year minimum) FP within a CHC providing full service longitudinal care is about 800 newly attached patients.
The FP will also carry out or support preventative care through call backs, screening and risk assessments, as well as contribute to population health programs by providing education to individuals, families, and communities. The FP with health care experience in all demographics, and with knowledge of elder care and mental health, will collaborate with the interdisciplinary team to provide full service primary health care.
Large Interdisciplinary Team
The team includes a director (1 FTE), a medical lead physician (0.2 FTE), contracted family physicians (FP 2.8 FTE), contracted nurse practitioners (NP 2.5 FTE), registered nurses (RN 3 FTE), LPN (0.5 FTE), contracted dietitian and occupational therapist (each 0.2 FTE), mental health clinician (1 FTE RSW), population health and wellness coordinator (1 FTE) and medical office staff (3.5 FTE).
Background
The CHC reports to Island Health, partners with the Victoria Primary Care Network, and has membership with the BC Association of CHCs. With a new renovated primary care space, the interdisciplinary team with full staffing will provide shared longitudinal care for 4500 intergenerational patients from the service areas of Gordon Head – Shelbourne (4115) with a population of ~40,000, and Oak Bay (4114) with a population of ~20,000. Priority attention will be provided to seniors/elder care and people with mental health conditions. LCS is a long-standing not-for-profit organization governed by an elected Board of Directors with exemplary accreditation status in the provision of long term care and assisted living.
