This is a Temporary Full-time position up to approximately eight (8) months.
Sinai Health is looking for a Social Worker to support our Mount Sinai Academic Family Health Team (MSAFHT) Department at our Mount Sinai Hospital Campus. This position is specifically to support our mental health program for our patients including children and youth under the age of 18.
The Social Worker, MSAFHT provides a range of social work services including assessment, crisis intervention, counseling and community referrals. The Social worker works collaboratively with the team and provides consultation to the team on managing issues that are psychosocial, emotional and mental health in nature related to our perinatal population.
To define and develop a culture and interprofessional primary health care practice model to enhance the delivery of patient centered care within an academic environment in the Mount Sinai community.
About The Mount Sinai Academic Family Health Team
The Mount Sinai Academic Family Health Team delivers comprehensive patient-centered primary care throughout every stage of life. Our goal is to help patients manage their health and enhance their overall quality of life. We do this by offering primary health care and several specialty and group programs.
We have locations in Toronto at the Granovsky Gluskin Family Medicine Centre and in Vaughan at the Sherman Health and Wellness Centre.
Family health teams bring several primary care providers into one care team. Working as a team means we can act as a primary point of care for patients and provide access to specialized supports in one location. The team consists of physicians, nurses, social workers, pharmacists and other specialized health-care providers. We have a special interest in perinatal care and the care of babies and children. Our services also include programs for diabetes management, mental health, nutrition, smoking cessation and geriatric care.
As a teaching clinic, we work closely with the University of Toronto Department of Family and Community Medicine to train medical students and resident doctors to provide the highest level of care. Our team values education, research and collaborative patient care.
In this role you will:
- Assess and provide social work diagnosis of psychosocial problems
- Provide individual, couple, family and group counseling and psychotherapy as well as host webinars
- Manage cases, including knowledge of and linkages to community resources
- Conduct psycho-education related to the prevention and management of mental health problems, including the facilitation of group session
- Assist in navigating service delivery networks and building relationships to find and secure required resources
- Advocate to establish and access needed resources
- Support patient self-management through the development of patient education resources/sessions
- Actively contribute to the education of all learners in the academic setting
- Implement standardized screening and assessment tools to support team delivery of mental health care
- Develop, manage and deliver programs alone or in collaboration with other professionals
- Consult with other professionals related to patient needs

- Case management rounds to refine treatment goals with the team
- Program development, planning, evaluation and clinical research in all aspects of the MSAFHT mental health program (e.g. assessment and care planning, crisis intervention and management)

Successful completion of a Masters of Social Work from an accredited educational institution

Currently registered and in good standing with the Ontario College of Social Workers and Social Service Workers

3 years of experience in direct client service within a community health care setting
Preferred

Previous experience with an electronic medical record (EMR), in particular PS Suite
-
Experience in program and role development

Experience supporting individuals coping with grief, trauma, and/or abuse
-
Experience working with populations facing social complexity (e.g., poverty, housing instability, systemic barriers) as well as individuals with complex mental health conditions
Skills and Knowledge

Demonstrated knowledge of contemporary theoretical frameworks and social work practice standards

Practical experience with evidence-based therapeutic modalities, such as Cognitive Behavioural Therapy (CBT) and Dialectical Behaviour Therapy (DBT)

Knowledge of community resources and a proven ability to make effective referrals to appropriate services

Demonstrated clinical skills working with a broad range of client groups and their families

Able to function in a fast paced, changing environment
