POSITION SUMMARY

Under the direction of the Health Manager, the Community Care Nurse (CCN) is responsible to coordinate and assist in the implementation and evaluation of the home and community Care, adult in-home care, discharge planning from hospital, care coordination between client & IHA, chronic disease program and various other patient care services.
The Community Care Nurse delivers and assists in the program to support people in their homes during times of acute or chronic health issues. This includes an intake process, assignment of care workers and ongoing supervision of program workers. The Community Care Nurse also provides nursing care to clients of the program such as, catheter insertion, wound care, foot care, and nursing assessment. The Community Care Nurse works with family members and community resources to ensure Elders and others with chronic health conditions are cared for safely and respectfully.
The CCN provides education to individuals and groups in the community with a focus on chronic disease prevention and treatment. This education is facilitated through family meetings, public presentations and support group meetings.
The CCN performs specialized nursing tasks including assessment for health conditions such as diabetes and heart disease by using appropriate screening devices and attends specialized hospital appointments at families and client’s request.

EDUCATION/EXPERIENCE

  • Bachelor of Nursing Degree from a recognized University
  • LPN required RN preferred
  • Current registration with BCCNM
  • Foot and Wound care experience
  • Equivalent combination of education, training and experience may be considered

REQUIREMENTS

  • Valid Class 5 Driver’s License and Acceptable Driver’s Abstract
  • Maintains a high level of confidentiality and uses discretion on all matters relating to the affairs of the Westbank First Nation
  • Acceptable Criminal Record check with Vulnerable Sector search to be obtained through the province
  • CPR – C or equivalent

JOB DUTIES/RESPONSIBILTIES

  • Coordinates the administration of the Home and Community Care, Adult In-Home Care, Diabetes and Chronic Disease programs
    • Stocks up wound care dressing trolley and home care bag supplies
    • Maintains an inventory of medical supplies, and materials
    • Contributes to ensuring a culturally appropriate lens is applied to programs and activities
    • Maintains copies of all teaching documentation
    • Maintains confidential records of client interactions within prescribed guidelines
    • Assists in maintaining the confidentiality of all interactions and records
    • Conducts all duties in keeping with all relevant legislation, policies, and procedures
  • Provides health support and basic medical services to individuals, families, and groups in the community as directed
  • Provides community education, support, and activities as directed
  • Provides specialized nursing services
  • Supports Home Support Workers and Homemakers
  • Maintains a professional status within the health team
  • Liaises with community agencies and support groups
  • Performs all duties and responsibilities in accordance with the Westbank First Nation policies, standards, and procedures
  • Performs other duties and responsibilities as necessary in the performance of the position and as assigned by the Health Manager

  • The incumbent must maintain strict confidentiality in performing the duties of the Community Care Nurse. The incumbent must also demonstrate the following:

    • Possess cultural awareness and sensitivity
    • Have a flexible schedule with availability to work evenings and weekends
    • Be consistent and fair

The median hourly wage for this position is $39.23
