About the role

Job Description Job Summary The Clinical Case Coordinator is a member of the interprofessional team. This role provides case management services to youth admitted to SAYC during their admission to the facility. This Clinical Case Coordinator provides support to youth through their admission, provides individual and group psychotherapy and identifies and coordinates internal and external resources for youth and their families during their admission. Additionally, the clinical case coordinator will coordinate the intake process in collaboration with other members of the Intake Committee.

Duties & Responsibilities Case management to coordinate and support services for youth and their families while at SAYC.

  • Identifying, developing and coordinating appropriate discharge plans for youth be collaborating with internal and external stakeholders.
  • Verbal communication and writing documentation that addresses youth’s care and treatment.
  • Coordinating meetings and appointments as mandated and/or as needed.
  • Individual and group psychotherapy for youth as assigned by supervisor.
  • In collaboration with the other clinical case coordinators and on a rotating basis coordinate the intake process, including initial contacts, initial assessment and triage, application review, screenings, scoring of standardized instruments and making recommendations regarding admission priorities.

Qualifications

  • Bachelors of Social Work (BSW) required. Individuals with Social Service work diploma and 5+ years of experience will be considered.
  • Eligible for registration with Ontario College of Social Workers and Social service Workers (OCSWSSW).
  • Excellent written and verbal skills.
  • 1-5 years of experience in related setting.
