Yurek Specialties Limited is a state-of-the-art processing facility in London, ON, which features the latest equipment to support the processing, production, and delivery of medical supplies and infusion equipment, along with related supplies, to the Elgin, Norfolk, Oxford, London-Middlesex, Huron, Perth, Grey, Bruce and Grand River counties.
We are currently looking for a dynamic and personable full-time Pharmacist to join our team at Yurek Specialties Limited in London for a 12-month contract.
Summary
The Pharmacist provides comprehensive pharmaceutical care for our patients and acts as a resource for families, contracted organizations (Ontario Health at Home); and professionals including nurses, dietitians and doctors. Pharmacists are accountable for ensuring our patients understand their medication therapies by counselling patients and working to full scope of practice in the expanded roles of the Pharmacist.
Shift Schedule & Hours of Work:
- Pharmacists are part of a rotation where they can be scheduled to work Monday through Friday from 8:00am to 9:00pm, and weekends 9:00am to 6:00pm.
- We are a 365 day a year operation, employees are required to be available to work weekends and holidays
- Pharmacists are also part of an on-call rotation
Responsibilities
- Clinical monitoring, care planning, and assessment of patients’ medication therapy.
- Programs and checks infusion pumps; troubleshoots issues and concerns with nurses, patients, and caregivers.
- Develops and maintains sterile compounding mixture worksheets as per OCP/NAPRA guidelines.
- Responds to and acts as necessary to fulfill after hours requests.
- Assesses IV orders for accuracy, appropriateness and determines the optimal infusion mode in collaboration with the prescriber.
- Assesses and processes Total Parenteral Nutrition (TPN).
- Assessment and Provision of Medical Assistant in Dying (MAiD) medication (pharmacist can object as per legislation exemption).
- Supervises IV intake orders ensuring seamless and accurate communication with providers and OH@H.
- Ensures appropriate infusion equipment/device are chosen for each IV order.
- Acts as a drug & infusion therapy resource for patients, physicians, nurse practitioners, dieticians, nurses, and other health care providers.
- The pharmacists are required to:
- Counsel clients on the safe use of medications and equipment
- Supervise and educate students
- Arranges for specialized training as required, for approval by Senior Management.
- Participates in conducting or arranging Continuing Education events as needs are identified or required.
- Completes all quality control functions and quality improvements tasks assigned
- On call responsibilities
Requirements
- Holds a Bachelor of Pharmacy Degree or Pharm D.
- Three (3) months experience in Kroll dispensary software an asset.
- Current Licensure and in good standing with the Ontario College of Pharmacists
- Experience in infusions and sterile processing preferred, or willingness to be trained
