Your Opportunity:

This is an exciting opportunity to be part of a dynamic and innovative interdisciplinary team that provides services to community dwelling clients who have complex rehabilitation needs. The Community Rehabilitation Interdisciplinary Service (CRIS) is seeking an energetic, creative and compassionate Social Worker II to provide support for clients with complex and neurological conditions. This position supports services offered at all three CRIS locations (Mother Rosalie Services Building at Misericordia Hospital, the Kaye Edmonton Clinic, and the Strathcona Community Hospital). The successful candidate will have the opportunity to work closely within an interdisciplinary team to provide assessment, intervention, screening, education, case management/coordination, and discharge planning services for clients/families receiving services from the CRIS clinic. Services provided may be 1:1 or in a group. The Social Worker II collaborates/consults with a variety of individuals/partners including clients/families, physicians, physical therapists, occupational therapists, speech-language pathologists, recreation therapists, therapy assistants, psychologists, and various internal and external partners/stakeholders to help enhance client outcomes and quality of life.

Description:

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:

Completion of bachelor's degree in Social Work.

Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

Current BCLS-HCP certification is required. Proficiency in computer applications including word processing, email, internet use, and electronic charting is essential. Strong assessment, communication, organizational, problem-solving, counseling, and group facilitation skills are required. Must work independently with sound judgment and collaborate effectively within an interdisciplinary team. A strong commitment to patient- and family-centered care is essential, along with the ability to foster partnerships that enhance outcomes. The role involves extended periods of sitting and cognitive load while delivering virtual care via Zoom and Connect Care. Physical demands may include transporting equipment (20–50 lbs) and bending/rotation during clinical duties. Recent experience in case management or service coordination for clients with diverse rehab needs is required. Experience working with patients with complex rehabilitation needs, knowledge of community and support services, and recent interdisciplinary team experience are all required. Experience with Connect Care scheduling and EMR, and training in HealthChange Methodology™ are also required.

Preferred Qualifications:

Experience with diverse cultural groups and new immigrants, applying cultural humility and adapting care to varied needs. Skilled in supporting care transitions across settings. Knowledge of trauma-informed care and harm reduction. Experienced in cross discipline mentoring, quality improvement, and research.
