Behavioral Health Clinician - Temporary Part - Time (0.6 FTE)
Location: Strathmore
Position Summary:
The Behavioral Health Clinician will work as part of a team providing a broad range of services to patients and their families residing in CRPCN geographical boundaries. The position will require collaboration with Primary Care Providers, CRPCN staff and community partners. These services are intended to foster optimal health for patients in the CRPCN geographical area by addressing the issues that can have an adverse impact on health and require experience in crisis intervention, biopsychosocial assessments, counseling, and may require resource navigation. The Behavioral Health Clinician will work with a high degree of autonomy and judgement
Key Responsibilities:
· Biopsychosocial assessment aimed at the early detection of a wide range of issues including but not limited to any type of abuse, adjustment to illness, addictions, anxiety, stress, depression, and risk to self and/or others
· Short-term (5-6 sessions) of counseling
· May require navigating resources, such as local services, provincial and federal benefits.
· May require safety planning for issues such as domestic violence, risk for suicide or homicide.
· Appropriate documentation on patient charts
· Collection and reporting on workload statistics
· Preparing for, presenting, and evaluating workshops
· Communication, consultation and collaboration with other social workers, doctors, other health providers and community services to facilitate patient care
· Advocacy for the rights of patients to access needed services
· Attend relevant community and professional meetings
Educational Requirements:
· A degree in social work, psychology, counselling, or another related field
· Registered and/or fully licensed and in good standing with a professional regulatory body
· Primary care, behavioral health training is preferred
· Health change methodology is preferred
Experience and Skills Required:
· 3-5 years recent experience preferred
· A demonstrated ability to work with people of all ages
· Experience providing crisis intervention, working with acute and chronic mental health populations and addictions preferred
· Demonstrated knowledge of community resources preferred
· Current knowledge of all relevant acts and legislation such as child, youth and family enhancement act, privacy act etc. is preferred
Core Competencies:
· Excellent written and verbal communication skills
· Excellent interpersonal and conflict resolution skills
· Proficient in the use of technology (i.e., MS Office, electronic medical records, cell phones)
· Knowledge of and adherence to clinical guidelines
· Comfortable working with various health care providers (both internal and external to the PCN)
· Ability to manage a varied and demanding caseload.
Working Conditions:
· Vulnerable population and criminal record check required
· Valid driver’s license & access to reliable vehicle required
· Medical clinic and/or office environment
· Ability to work independently and with limited supervision
· Occasional travel for training or to CRPCN work sites required
· Typical working hours with some flexibility
