Waypoint Centre for Mental Health Care is a 315-bed academic and teaching hospital that provides specialized mental health, addiction and geriatric care. Located on the shores of Georgian Bay, we serve some of the most complex and disadvantaged individuals in Ontario and are the sole provider of high-secure forensic mental health services in the province.
The Manager of Rehabilitation Services is currently seeking a Rehabilitation Counsellor- NSYWH. As part of the North Simcoe Youth Wellness Hub (NSYWH), the Rehabilitation (Intake) Counsellor will work collaboratively as a member of the Youth Wellness Team to receive all incoming requests for service and aid in the implementation of the care continuum model to support youth needs aligned with the values and mandate of Youth Wellness Hubs Ontario (YWHO). Further, this role will assist youth in their recovery through system navigation and coordination of participation in a range of services available through the NSYWH

KEY ACCOUNTABILITIES:
- Interview youth, prepare case histories, assess interests, strengths and barriers, and support youth engagement with appropriate NSYWH services, using YWHO intake processes and standardized measures.
- Develop and implement counselling and/or skills-based services using the YWHO care continuum model to assist youth in determining goals using evidence-based practice.
- Develop interventions in collaboration with registered interdisciplinary team members (e.g., Mental Health Counsellor or Care Coordinator) during complex youth presentations (e.g., high risk), as well as when navigating to medium and high priority services within the care continuum model.
- Liaise with the NSYWH partners using the YWHO care continuum model to provide referrals and proactively/warmly transfer youth to YWHO partners and/or external service providers.
- Evaluate the effectiveness of interventions and movement towards goals, including follow-up on results of services while supporting measurement-based care through the use of the My Wellness Passport system (for the Youth Wellness Team, partners, and other service providers). This may involve facilitating continued engagement of youth at YWHO to ensure a continuity of care between all service providers.
- Provide public education to a variety of NSYWH partners and/or external groups, and consultation with other Youth Wellness Team members regarding the NSYWH services. Further, the incumbent will be required to connect with, and build professional relationships with relevant stakeholders.
- Contribute to departmental and organizational team processes including involvement in special projects, service development, planning, promotion and evaluation to ensure alignment of the Rehabilitation (Intake) Counsellor role with the goals of the Youth Wellness Team, and overall values and mandate of YWHO
- Create and maintain an inclusive and welcoming space for youth to access services available through the NSYWH.
- Work from a harm reduction and youth-centered approach.
- Provide technology-based (online, phone) and in-person service, as deemed appropriate by the youth accessing services.
REQUIREMENTS/QUALIFICATIONS:
- Diploma or Degree in a health-related discipline required
- Class “G” Driver’s license in good standing required
- Preference for a regulated healthcare professional licensed to practice in Ontario
- 3 years of demonstrated experience in the mental health rehabilitation sector required
- Results oriented work ethic to work independently and undertake tasks needed to accomplish work objectives and deliver quality, consistent and timely results
- Demonstrated leadership, conflict resolution, interpersonal and organizational skills
- Proven written and verbal communication skills
- Proven ability to adapt readily to change and reflects an optimistic and positive attitude
- Models and promotes core ethical practice, Waypoint Values (Caring, Respect, Accountability, Innovation)
- Preference for proficiency in French/English language skills.
