About the role
Job Summary:
We are seeking a dedicated and compassionate Registered Nurse (RN) to join our healthcare team in Ontario. You must be licensed In BC, However do not require to live in BC. The RN will be responsible for conducting comprehensive mental health assessments, developing individualized care plans, and providing ongoing follow-ups to monitor patient progress. The role involves collaborating with interdisciplinary teams, supporting patients with diverse mental health needs, and ensuring high-quality, patient-centered care.
Key Responsibilities:
✅ Assessments & Care Planning:
- Perform comprehensive mental health assessments, including evaluating emotional, psychological, and physical health status.
- Identify patient needs, formulate nursing diagnoses, and create individualized care plans.
- Conduct risk assessments for suicide, self-harm, or violence, and implement appropriate interventions.
✅ Follow-ups & Monitoring:
- Provide regular follow-up visits to monitor patient progress and response to treatment plans.
- Adjust care plans as needed, based on clinical observations and patient feedback.
- Collaborate with physicians, psychiatrists, and other healthcare professionals to ensure continuity of care.
✅ Patient Support & Advocacy:
- Offer therapeutic support, crisis intervention, and mental health education to patients and families.
- Promote patient advocacy by ensuring access to appropriate mental health resources and services.
- Provide emotional support and coping strategies to patients dealing with mental health challenges.
✅ Documentation & Reporting:
- Maintain accurate and up-to-date patient records, including assessment notes, care plans, and follow-up reports.
- Ensure all documentation adheres to regulatory and organizational standards.
- Report significant changes in patient conditions to the care team promptly.
✅ Collaboration & Education:
- Participate in case conferences and interdisciplinary team meetings.
- Educate patients and families about mental health conditions, treatment plans, and self-care strategies.
- Contribute to quality improvement initiatives and ongoing professional development.
Qualifications & Requirements:
- Registered Nurse (RN) in good standing with the College of Nurses of BC
- You must be licensed In BC, However do not require to live in BC.
- Virtual Healthcare Experience
- Minimum of 2 years of experience in nursing, community nursing, or a related field.
- Excellent communication and interpersonal skills to engage with patients, families, and healthcare teams.
- Ability to work independently and manage a flexible caseload.
Preferred Skills:
- Experience with cognitive behavioral therapy (CBT), dialectical behavior therapy (DBT), or other therapeutic approaches.
- Knowledge of electronic health records (EHR) systems.
- Certification in Mental Health First Aid or related credentials is an asset.
