About the role
Woolwich Community Health Centre (WCHC) requires a relief (0.1) clinical nurse (RPN) to provide day and evening coverage for our St. Jacobs, Wellesley, and Linwood sites. The Clinical Nurse functions as a member of the inter-disciplinary team by providing a range of nursing services essential to client care.
Requirements:

Registered Practical Nurse (RPN) with current Certificate of Registration with the College of Nurses of Ontario

2-3 years inter-disciplinary nursing experience in a community-based primary care setting is preferred; new graduates are welcome to apply.

Knowledge of community resources/services to facilitate referrals for primary health care clients

Ability to manage multiple and complex demands

Commitment to working collaboratively in a team environment

Knowledge of/experience with health issues facing seniors, rural residents, youth, and families with young children
-
Facility with PS Suite EMR considered an asset
-
Excellent telephone and email communications skills, triaging, assessments, venipuncture, injection, wound care, issue management and strong interpersonal skills within the context of an inter-disciplinary team setting with clients and community members
Hours and location of work vary, based on coverage needs at any of the facilities.

- Option for enrolment in Healthcare of Ontario Pension Plan (HOOPP)
