Lumenus is hiring an Integrated Response Facilitator.

Reporting to Director of System Services, this newly created role of Integrated Response Facilitator is responsible for facilitating case resolution for adults with a developmental disability, complex mental health and behavioral issues (Dual Diagnosis), in collaboration with the Integrated Response Committee.

The Integrated Response committee (IRC), will carry out the new provincial mandate of Service Solutions. The IRC is a community lead committee that brings together high level managers and decision-makers from a cross-section of organizations from multiple social service and health sectors.

Working with partners of the Developmental Sector and additional stakeholders, service team and the compilation of data and lessons learned through service resolution the Facilitator is responsible for developing processes, partnerships and pathways needed to support this new initiative.

KEY RESPONSIBILITIES:

  • Provide system coordination and facilitate an integrated response in responding to the needs of individuals with Dual Diagnosis through the following skills and activities:

o Provide a point of contact for service coordinators, community partners and representatives of the DSO for service solutions

o Screen and assess referrals to IRC for appropriate fit

o Ensure that the referral form, as prepared by the referral source is complete;

o Ensure that all avenues to find a community service solution have been explored by the case manager

o Ensure that presentation material/referral package is accurate and complete

o Support the case manager in the case presentation to the IRC

o Collaborate with the chair of the IRC to facilitate meeting process

o Follow up on outcomes and recommendations from the IRC by providing advanced sector specific knowledge –Dual Diagnosis, Mental Health and Complex Behaviours, Collaborative Tables

o Establish streamlined referral pathways, relationships and processes for partner agencies and ad-hoc partners

o Refer and liaise with other service providers as required

o Provide advocacy, build capacity in planning with regards to crisis prevention and intervention, for complex cases

o Assist service providers to identify and develop comprehension plans of care on behalf of clients

o Assist service providers to identify goal areas and specific objectives on behalf of clients within the scope of IRC

o Provide support, education, and information to partnering agencies

o Identify problem-solving strategies for resolution of immediate needs

o Working closely with all disciplines participating in intake, assessments/screening, information gathering, environmental scans and data collection.

o Liaise with cross-sectoral referring pathways, and other service providers to coordinate supports for service resolution

o Facilitate local case resolution through active consultation and participation in collaborative system efforts where the focus is to address complex or critical service user needs and supporting individuals who require a multi-service approach

o Participate in evaluation of partner, client satisfaction

o Gather data and identify service gaps where there are community development needs and system improvements required

o Work with Integrated Responsive committee members to escalate system gaps to the local planning tables and to funders

o Providing education and training to community providers on a variety of topics (i.e. Dual Diagnosis, behavioral strategies etc.).

EXPERIENCE/KNOWLEDGE:

  • Bachelor or Master in Health and Human/Social Services or equivalent experience and a minimum of 5 years direct clinical experience in the area of Developmental Disabilities, complex mental health and addictions

  • Demonstrated experience coordinating and supporting projects

  • Excellent planning, organization, problem-solving, decision-making and liaison skills, and leadership skills

  • Excellent knowledge and understanding of developmental disability, mental health and addictions community resources

  • Excellent verbal and written communication skills

  • Ability to collect and communicate data to various stakeholders

  • Strong relationship management and community engagement skills

  • Computer proficiency in MS Office Products, word processing, spreadsheets and e-mail.

ADDITIONAL REQUIREMENTS:

  • Satisfactory clearance under the Vulnerable Sector Screening process.

  • Ability and willingness to work flexible hours to meet with the individuals and their community supports.

  • Valid Ontario G class driver’s license and a vehicle as travel within the city may be required.
