Job Description:

The Senior’s Services Physiotherapist is accountable for the following:

Qualifications

  • Candidate must be a graduate of an accredited School of Physiotherapy
  • Registration with the College of Physiotherapists of Ontario
  • C.P.A member preferred
  • GPA training preferred
  • ADP Authorizer preferred
  • Experience with treating medically complex older adult patient population preferred
  • Geriatric experience preferred
  • Strong critical thinking, decision making, problem solving and conflict resolution skills
  • Commitment to evidence-based practice and quality improvement
  • Commitment to patient centered care
  • Superior interpersonal and communication skills
  • Demonstrated effectiveness working independently as well as collaboratively with the interprofessional team
  • Curren certification in Basic Cardiac Life Support preferred
  • Knowledge of community programs and resources for the older adult population an asset

Job Description

  • assessment of geriatric population in multi-disciplinary clinic with geriatricians
  • triaging patients through initial assessment according to falls risk and functional ability to determine eligibility to participate in 6 week Strong and Steady falls prevention program
  • Awareness of alternative options/programs and provision of these options to patients should they not fit eligibility criteria of above
  • community resourcing for clients for falls prevention and advocacy/referral to appropriate community programming
  • occasional coverage of exercise classes and education sessions for the Strong and Steady falls prevention circuits in community centres (northwest and central Mississauga)
  • responsible for supervision and oversight of OTA/PTA x2 and volunteers
  • re-assessment of clients at completion of circuit participation
  • travel to community centres mandatory – access to vehicle required
