Title: Wellness Navigator

Salary: $88,948 - $104,644 - $120341 per year (pay range explained below)

Contract: Term for 1.5 years

Hours: 37.5 hours (core hours 9am-3pm Mon-Fri)

Location: Fraser Salish Region, BC, located on the ancestral territories of the Stó:lo¯ Nations – Hybrid

About The Role As Wellness Navigator, you will be a driving force in shaping healthier, more vibrant, and self-determined First Nations communities in the Fraser Salish Region. You will work closely with individuals, families, and communities to enhance health and wellness, supporting those living with chronic conditions through personalized wellness planning and increased health literacy. You’ll champion a holistic, person-centered approach to health and social systems, helping to map services and guide wellness journeys across the full spectrum of care. Your role will also be collaborating with Fraser Health Authority as part of a regional network grounded in joint planning and shared goals.

About You To be successful in this role you will have/be –

Education

  • A Bachelor’s degree in a relevant field or a equivalent combination of education and experience.

Occupational Certification

  • Certifications in First Aid Level 1 (with CPR), Hep B shot, and flu vaccine.

Experience

  • 5–7 years of significant, recent, and relevant experience, particularly in regional health authority systems (such as Fraser Health Authority), working with First Nations individuals, communities, or organizations on health and wellness issues.
  • Experience coordinating patient care across various health issues, with a focus on chronic care management and supporting individual health and wellness plans.
  • Proven ability to map health and social systems and assets to support holistic, person-centered care and community wellness.
  • Hands-on experience developing and guiding the approval of health and wellness plans, including for individuals managing chronic conditions.

Knowledge

  • In-depth knowledge of the BC healthcare system, First Nations health landscape, and the health and wellness needs of Aboriginal/First Nations communities.
  • Strong understanding of person-centered care, with the ability to deliver holistic, integrated, and culturally respectful services.
  • Comprehensive knowledge of community, public, and primary health care systems.
  • Knowledge in trauma-informed practice and motivational interviewing.

The Pay Range is the minimum and maximum annual salary based on full time equivalent hours.

Incumbents are typically hired, transferred or promoted between the minimum and midpoint of the range based on their knowledge, skills, abilities and experience in relation to the role requirements.

The top 10% of the pay range is for the incumbents who are industry experts in the job with the combination of exceptional experience and competencies needed to perform all duties and responsibilities at a superior capability level. Internal employee compensation will be determined as per the FNHA Employee Total Compensation Policy.
