HGMH provides acute care, 24-hour emergency services, outpatient services and rehabilitation to over 41,000 residents every year. Position Purpose
This role provides expertise in key program areas including Infection Prevention and Control (IPAC), occupational health and specialized patient programs. Through collaboration, education, and evidence-based practice, the nurse supports both patient outcomes and organizational health, ensuring alignment with regulatory standards and hospital priorities.
Accountabilities

Infection Prevention and Control (IPAC)
- Develops and implements an annual surveillance plan, approved by the Infection Prevention and Control Committee.
- Conducts ongoing surveillance and documentation of infections in patients and personnel; prepares and presents reports to the Infection Prevention and Control Committee.
- Investigates infection clusters and leads outbreak management procedures in accordance with established protocols.
- Develops and updates isolation practices, techniques, and procedures in alignment with current standards and regulations.
- Participates in quality and performance improvement activities by identifying infection risks and implementing mitigation strategies.
- Inspects the clinical environment and observes staff practices to identify infection hazards and ensure compliance with infection control policies.
- Compiles and analyzes data on infections and antibiotic usage as part of routine surveillance and antimicrobial stewardship.
- Monitors patient admissions and placements to support optimal infection control practices.
- Coordinates Infection Prevention and Control Committee meetings and ensures hospital-wide dissemination of recommendations.
- Reports all in-house patients with communicable diseases to the Eastern Ontario Public Health Unit and maintains accurate records.
- Conducts IPAC education and education programs for staff, physicians, and volunteers; develops appropriate informational materials.
- Makes recommendations on equipment and supply purchases to ensure IPAC standards are met.
- Participates in Accreditation activities and leads relevant documentation and evidence related to IPAC standards.
- Serves as Chair of the IPAC Committee and participates in other assigned hospital committees.
- Approves construction projects from an infection prevention and control perspective

Occupational Health & Employee Wellness

Coordinates follow-up and treatment for employees exposed to blood, body fluids, or communicable diseases.

Leads the development and implementation of employee vaccine programs (e.g., influenza, hepatitis B).

Oversees and maintains progressive programs in employee health and wellness in collaboration with health and safety staff.

Reviews and updates employee health policies related to infection prevention in consultation with the IPAC Committee.

Supports the development and promotion of staff wellness initiatives, including education, engagement, and mental health resources.

Administers and maintains the N95 Fit Testing Program, ensuring annual compliance and proper documentation for applicable staff.

Specialty Clinical Programs

Coordinates and delivers the Smoking Cessation Program to patients and staff, providing tailored education and follow-up support.

Leads the Hospital Elder Life Program (HELP) by supporting the implementation of best practices for delirium prevention.

Provides wound care consultation, supports staff education, and ensures evidence-based wound assessment and treatment planning.

Patient Care and Transition Support
-
Conducts timely discharge follow-up phone calls with patients to assess recovery, reinforce discharge instructions, and address any emerging concerns.

Escalates clinical issues to appropriate departments and contributes to improved patient transitions and reduced readmissions.

Collaborates with interdisciplinary teams, managers, and educators to promote safe, quality care and wellness initiatives.

Participates in the development and review of policies and procedures aligned with regulatory requirements and hospital standards.

Maintains accurate, timely, and confidential records in all areas of responsibility.

Supports other duties as assigned in alignment with hospital goals and priorities.
Requirements
- Current registration with the College of Nurses of Ontario (CNO) in good standing.
- Minimum of 3–5 years’ clinical nursing experience, preferably in acute care or public health.
- Specialized training and/or certification in Infection Prevention and Control (CIC certification or equivalent) preferred.
- Experience with outbreak management, surveillance, and reporting to public health authorities.
- Knowledge of relevant legislation and standards (e.g., PIDAC, PHO, Public Hospitals Act, OHSA).
- Experience in occupational health practices, including immunization programs, exposure follow-up, and wellness initiatives.
- Wound Care Champion Program (WCCP) for Regulated Health-Care Providers certificate.
- RNAO Best Practice Champion certification
- Knowledge of geriatric care best practices, and smoking cessation support strategies.
- Proficiency in completing and documenting N95 fit testing and managing related records.
- Strong interpersonal and facilitation skills to support education, wellness initiatives, and staff engagement.
- Excellent organizational and time management skills to manage multiple concurrent programs.
- Proficient in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and electronic health records.
- Experience supporting accreditation activities and compliance with regulatory standards.
- Excellent attendance record.
- Bilingualism is preferred
