About the role

Reporting to the Director, Patient Care Services, the Patient Flow Coordinator is responsible for managing and coordinating patient movement across the Ottawa and Brockville Forensic Treatment Units (FTUs). The role support timely, safe, and coordinated transitions into, within, and out of the Integrated Forensic Program.

RESPONSIBILITIES

  • Coordinate patient flow into the FTUs from external sources, including courts, correctional institutions, and community-based re-admissions.
  • Ensure all intake processes are aligned with legal, operational, and clinical requirements.
  • Act as a liaison with correctional services, legal authorities, and clinical leadership to facilitate timely admissions.
  • Maintain accurate records related to referrals, admissions, discharges, and transfers.
  • Monitor system metrics to identify trends or areas requiring intervention.
  • Prepare data summaries or dashboards for operational decision-making and reporting purposes.
  • Maintain up-to-date awareness of bed availability, service demands, and clinical capacity.
  • Align patient discharges with upcoming admissions to optimize system flow.
  • Coordinate with receiving programs or services to ensure smooth patient transitions.
  • Maintain a centralized and current waitlist for the FTUs.
  • Track referrals, prioritize based on risk, acuity, legal status, and wait time.
  • Identify delays or systemic issues impacting patient movement, including ALC (Alternate Level of Care) concerns.
  • Escalate unresolved issues to operational or physician leadership for resolution.
  • Serve as the central coordination point for patient flow issues within the forensic program.
  • Communicate with clinical teams, legal and correctional partners, and operational leaders on flow-related matters.
  • Participate in program planning and flow optimization initiatives.
  • Work in a manner that complies with staff and patient/client safety practices, policies and procedures of The Royal.

  • Support the department/unit by performing other duties as necessary to uphold organizational standards and contribute to both clinical and administrative functions, where applicable.

QUALIFICATIONS

  • Undergraduate degree or Post-Secondary Education in Health Administration, Nursing, Social work or related health or human services field. Equivalent experience in a similar coordinator role may be considered.
  • Minimum 3 years’ experience in patient flow coordination, health system navigation, or clinical operations.
  • Experience within forensic, mental health, or correctional settings is strongly preferred.
  • Familiarity with legal frameworks and procedures related to forensic patient care (e.g., court orders, secure transfers) is an asset.
  • Strong organizational and time management skills required for tracking multiple admissions, discharges, and transfers across two campuses, while maintaining up-to-date flow and waitlist data.
  • Excellent verbal and written communication skills Ability to manage competing priorities in a complex environment that are Critical for balancing real-time flow issues, urgent legal admissions, and system bottlenecks in a dynamic forensic setting.
  • Skilled in collaborative problem-solving and cross-functional coordination.
  • English level A- is mandatory in oral expression, comprehension, reading and writing. Bilingual (French/English) is considered an asset.
