Job Description:

This role (Clinical Coordinator) is assessed as a Roamer position requiring minimal 3 days in the office unless business demands require.

Job Summary:
The Clinical Coordinator is responsible for completing or coordinating clinical intake functions, providing administrative support to clinical team throughout clinical service provision, supporting the maintenance of the client health record throughout clinical service provision, liaising with community engagement lead as needed throughout intake functions and clinical service provision.

Duties & Responsibilities (not limited to):

  1. Clinical Intake
  • Creating and/or updating client health record as needed.
  • Tracking referrals for service.
  • Completing initial call with families across service regions.
  • Providing and/or reviewing clinical recommendations to/with prospective client (service stream specific).
  • Coordinating the schedule for services with the client and clinical team.
  • Liaising with referral sources re: recommendations, schedule, selection of specific service.
  • Reviewing required documentation including: clinical agreement/consent(s), welcome (clinical intake) package.
  1. Administrative Support to Clinical Team throughout Clinical Service Provision
  • Sending documents to families as needed (e.g., consent forms, reports, program letters to families).
  • Administrative supporting to weekly intake meetings.
  • Supporting the provision of pre-service, in-service and post-service client satisfaction surveys.
  • Monitor and report on capacity in programs for additional client intakes.
  • Assist with other coordinator and administrative tasks across services SSP supports as needed (e.g., parent education stream).
  1. Liaise with Community Engagement Lead
  • Support workflow efficiency, development.
  • Support capacity/vacancy mgmt. and conversion tracking.
  1. Support Maintenance of Client Health Record
  • Maintain/update demographic information.
  • Ensure signed reports uploaded to tasks.
  • Ensure tasks, contact authenticated and appointments resolved.
  • Ensure consents are up to date.

Qualifications:

  • Minimum of one year experience working in a related position.

  • Completion of Community College Diploma or equivalent.**

  • Familiarity of Autism Spectrum Disorder, the OAP, ABA scope of practice.

  • Proficient in using advanced features of Microsoft Office Suite (Word, Outlook, Excel, and Access) and database structures.

  • Strong organizational skills and ability to track tasks based on level of completion.

  • Ability to work independently without supervision.

  • Preferred Social Services Worker or equivalent.
