REGENT PARK COMMUNITY HEALTH CENTREWe are a dynamic community-based health centre promoting social justice and health equity in the Regent Park neighbourhood and Downtown East area of Toronto. We operate out of several locations and provide a wide variety of services including: primary health care, chiropody, physiotherapy, health promotion, social work, community health, immigrant and refugee services, harm reduction and homeless programs, addiction and recovery services, Pathways to Education, infant and child development programs, parenting programs, and youth employment services.

Duties
Specific Duties Include:
- Direct Client Care:
o Conducting referral and intake;
o Assessing client nutritional status by gaining an understanding of food habits or preferences, socio-economic and cultural backgrounds, psychological factors and medical profile;
o Developing, implementing and evaluating individualized nutritional care plans, considering literacy level and relevant cultural and psycho-social factors;
o Ensuring continuity of care by maintaining up-to-date client records, maintaining clients on a regular basis, and case conferencing with other providers as required;
o Referring clients to other health and social professionals when appropriate;
- Health Promotion and Education
o Planning and organizing health promotion activities and workshops, liaising with other teams regarding these activities;
o Obtaining and evaluating nutrition education resources in clear language to meet the needs of clients;
o Planning, implementing and evaluating community nutrition education activities/groups and direct nutrition services;
o Encouraging community participation in activities; e.g., through focus groups, advisory committees.
o Participating in the development of multidisciplinary health promotion nutrition programs;
o Acting as a resource for Health Centre staff in the area of nutrition;
o Maintaining networks with peers and other related health professionals where appropriate.
Requirements
- University Degree accredited by Partnership for Dietetic Education and Practice (PDEP)
- Completion of an accredited internship and at least 2 years’ experience working as a Dietitian preferably in a community-based organization.
- Registered member of the College of Dietitians of Ontario
- Diabetes Educator (CDE) an asset
- Clinical skills in all aspects of assessing and developing a nutrition care plan.
- Expertise in community development, harm reduction principles, health promotion and the Social Determinants of Health
- Experience working with people from diverse cultures, races, ages, abilities, economic backgrounds, as well as those with substance use issues, and experience of homelessness
- Understanding of anti-oppression principles and experience doing social advocacy
- Experience in program planning, delivery and evaluation
- Ability to take initiative and work independently
- Certification in Intuitive Eating, HEAS, Craving Change and/or Motivational Interviewing as asset
- Ability to speak other languages an asset, including Vietnamese, Spanish, Somali, or Tigrinya
- Ability to work collaboratively within and across inter-professional team
- Organizational and time management skills
- Ability to work flexible hours
- Strong written and oral communication skills, and strong computer skills
- Knowledge of the Regent Park area and issues affecting the community
- Must be fully vaccinated for COVID-19 for the protection of clients and staff
