The Opportunity:
We are recruiting a MENTAL HEALTH CLINICIAN (Seniors Services)
- This is a regular full-time opportunity (35 hours per week Monday to Friday 9am to 5pm) based at 80 Waterloo Ave. Guelph.
Reporting to the Manager, Seniors Services, this position will provide mental health assessment, treatment support and intervention services for seniors and support to their caregivers who experience complex, persistent mental health, major neurocognitive disorders, and / or addictions issues in order to optimize functioning and quality of life. This role consists of assertive outreach, treatment-based care, in home assessments, care planning and execution of comprehensive mental health assessments within a person-centred framework. The Mental Health Clinician will collaborate within a team-based model of care. Collaboration with other disciplines, community services partners, facilitation of groups for clients and / or their caregiver and care planning with the team are an integral part of this role. The Mental Health Clinician works closely with geriatric system partners including Primary Care, Geriatric Psychiatry, Specialized Geriatric Services, Ontario Health at Home and various other community partners. This role has an advanced ability to identify risk, apply appropriate risk mitigation strategies and requires a solid understanding of what factors contribute to complex client presentations (e.g. adverse childhood experiences, developmental trauma, attachments and concurrent disorders). An asset is a solid understanding of substance misuse.
Qualifications:
- Masters level degree in social work, psychology, registered nurse or relevant health or social science field
- Registration with relevant college/registering body.
- 3 to 5 years of experience in the provision of geriatric comprehensive assessment and mental health service with older adults with medical, cognitive, mental health and addictions and behavioural complexities
- Skills, knowledge, and clinical practice with the administration and interpretation of geriatric assessment, consultation, care planning implementation and interventions to support the needs of the older adult and family dynamics
- Ability to adapt rapidly to changing risk situations
- Sound understanding and demonstrated knowledge of the Substitute Decisions Act, Health Care Consent Act and working knowledge of PHIPA and the applicability of this as an enable of safe quality care
- Demonstrated ability to flag issues of capacity
- Ability to provide education, and opportunities for collaboration to colleagues, community partners and colleagues regarding the provision of care to seniors’ with mental health issues and other complexities related to social determinants of health
- Demonstrated understanding and awareness of social, gender and cultural diversity supports that may be required in the support of an older adult
- The ability to work collaboratively in a multidisciplinary team – with internal team and external service partners.
- Valid driver’s license, vehicle and insurance as travel is required
