Primary Care Nurse Practitioner
Employment Status: Full-time permanent position
Work Hours: 35 Hours per week (occasional evening or weekend work will be required)
Compensation: $61.00 to $72.00/hour (commensurate on skills and experience, plus participation in
Benefits & HOOPP.)
Additional Compensation: Sign-on bonus of $3000.00 to $4000.00
Reporting to: Manager of Primary Care
Location: Scarborough, ON; (with flexibility to transition to/work at other sites as required).
Travel Requirements: A valid driver’s license and access to a vehicle are required
Job Overview:
We are seeking a Primary Care Nurse Practitioner to join our client’s team, a leading healthcare
provider in Scarborough, ON.
Job Duties:
Deliver holistic, client-centered care within the established scope of practice.
Perform assessments, interventions, ongoing monitoring, and follow-ups for both acute and
chronic conditions.
Conduct patient interviews and physical examinations, develop individualized treatment
plans, and incorporate evidence-based health promotion strategies.
Comply with the professional standards and guidelines set by the College of Nurses.
Collaborate effectively within an interdisciplinary healthcare team.
Engage in peer and chart audits as required.
Participate in case conferences to support comprehensive patient care.
Work effectively in a team environment to provide care to marginalized individuals,
including those with chronic illnesses, mental health challenges, and substance use
disorders.
Demonstrate a commitment to professional growth by engaging in development
opportunities and mentoring Nurse Practitioner or medical students.
Maintain required competencies through continuous learning and professional
development.
Ensure patient involvement in all aspects of their treatment and care plans.
Educate and support patients in managing treatments, minimizing side effects, and
establishing follow-up care.
Offer telephone support as needed to assist patients with their care.
Facilitate community education sessions based on availability.
Foster and maintain professional relationships with external agencies, educational
institutions, and healthcare organizations.
Participate in the organization's on-call schedule in alignment with funding requirements.
Pursue ongoing education and training opportunities to enhance clinical expertise.
Requirements:
Master’s degree in nursing with a Primary Health Care Nurse Practitioner certificate.
Active registration in good standing with the College of Nurses of Ontario (Extended Class).
Membership with the Nurse Practitioners' Association of Ontario (NPAO), including
additional professional liability coverage.
2–5 years of experience as a Nurse Practitioner, applying advanced assessment skills within
an interprofessional healthcare setting.
Paediatric experience is considered an asset.
Strong preference for candidates with Nurse Practitioner experience in a community
healthcare setting.
Prior experience in program development is advantageous.
Current certification in Healthcare Provider (HCP) CPR.
Willingness to join additional professional organizations, such as the Registered Nurses’
Association of Ontario (RNAO), to further enhance practice.
Proficiency in additional languages relevant to patient care is considered an asset.
Proven ability to work independently with strong self-direction and organizational skills.
Excellent interpersonal abilities, including strong written and verbal communication skills, as
well as effective conflict resolution capabilities.
