Job Type: 2 Part-time Positions (2-year Contract)
Location: 265 KERR ST**,** Oakville, ON
Late afternoon, evening and weekend work required.
Hours of work are subject to change based on program needs.
Hourly Wage: $22.00 – $24.00 per hour

Position Overview
This is a dynamic, hands-on role based within a supportive housing environment at 265 Kerr Street in Oakville, designed for older adults transitioning from shelter to permanent housing. The position plays a vital part in a holistic care model that supports individuals facing complex challenges, including medical comorbidities and mental health concerns—particularly those without strong social or family networks.
Residents in this program often encounter barriers to maintaining stable housing due to limited support systems and diverse health needs. By delivering integrated, wrap-around services that address both health and social care, this role contributes directly to improving housing stability and overall quality of life for vulnerable community members.
Role and Responsibilities
- Coordinates and supports the implementation of personalized activities and residential programming through:
- Establishing a good rapport and communicating effectively with team members and clients, acting as an advocate for each person supported.
- Planning and assisting in facilitation of leisure and recreational activities for all individuals.
- Performing regularly scheduled cleaning duties within the common area, warming kitchen, staff office, ensuring that health standards are maintained.
- Planning and carrying out activities of interest to clients, according to their goals.
- Remaining informed and up to date on individuals’ medical conditions, required supports, and associated precautions.
- Managing emergency situations as they arise, ensuring the safety and well-being of all individuals by following established protocols.
- Performing daily administrative tasks such as accurately documenting client interactions, updating records, maintaining logs in organizational databases, and ensuring timely communication of relevant information to the team.
- Actively contribute as a collaborative team member, upholding the Vision, Mission, and Values of the Agency, while engaging with stakeholders and community partners as needed.
- Ensuring all mandatory training and certifications are completed and kept current.
Qualifications
- Supportive Care Worker or Community Service Worker Certificate or Recreation Therapist diploma, Social Service Worker diploma and/or equivalent related years of experience.
- Current CPR, First Aid Certificate and/ or Mental Health Certificate an asset.
- Non-Violent Crisis Prevention, Assist, Motivational Interviewing and knowledge of House First Model is an asset.
- Effective communication skills, written and verbal.
- Ability to interact with others in a caring, tactful and courteous manner.
- Excellent time management skills with the ability to work independently with minimal supervision.
- Ability to work collaboratively and cooperatively with others as part of a team.
- Completion of a satisfactory Vulnerable Sector Screen (VSS) prior to employment and throughout employment is required.
