About the role

The Niagara Falls Community Health Centre (NFCHC) is a non-profit, community-governed organization dedicated to providing accessible, comprehensive health services at no cost to residents of Niagara Falls. NFCHC offers a wide range of programs including primary care, dental services, mental health counseling, nutrition guidance, occupational therapy, and specialized support for 2SLGBTQ+ individuals, refugees, and newcomers. The Centre emphasizes health promotion and community development through free workshops and outreach initiatives designed to address social determinants of health. Position Summary:

Working closely with the Director of Operations, and reporting to the Clinical Director, the Care for Kids Lead will be a valuable member of the pediatric team in supporting the organization’s strategic plan and vision. The Care for Kids Program Lead plays a key role in overseeing and advancing pediatric health services at Niagara Falls Community Health Centre. With a strong nursing background, this individual ensures high-quality, family-centered care while promoting health equity and accessibility for children and their families. This role requires strategic program management, clinical expertise, and collaboration with multidisciplinary teams to deliver comprehensive pediatric healthcare services.

Main Responsibilities/Duties:

Duties and responsibilities include, but are not limited to:

  • Lead the development, implementation, and evaluation of the Care for Kids program to meet the health needs of children 0-5 years in the Niagara Region.
  • Conduct routine well-baby exams which include immunizations and assessments.
  • Responsible to provide and lead health education and teaching while remaining informed of pediatric clinical guidelines and recommendations.
  • Provide point of care in other community settings.
  • Collaborate with community partners and other health organizations to establish pediatric care pathways throughout the Region.
  • Facilitate/coordinate health education and outreach initiatives focused on child wellness and preventive care.
  • Maintain clinical supplies and equipment with timely inventory checks and orders.
  • Maintain and oversee implementation of NFCHC’s Infection Prevention and Control Standards (IPAC) while staying informed on IPAC practices.
  • Responsible to maintain vaccine fridge. This includes maintaining temperature logs and reporting to Public Health.
  • Responsible to maintain inventory and order vaccines.
  • Represent the NFCHC at meetings and planning tables regarding pediatric care.
  • Participate in the development and monitoring of service and program indicators.
  • Identify areas for improvement or opportunities to implement more efficient processes.
  • Engage families in care planning and decision-making to foster a supportive healthcare environment.
  • Provide updates on quality initiatives, progress, and issues to the leadership team.
  • Lead communication with the Care for Kids team during pertinent updates.
  • Promote a culture of collaboration and respect across all disciplines and functions.
  • Contribute and support implementation of organization’s strategic plan and priorities.
  • Adhere to the expectations of NFCHC policies and procedures and operating procedures.
  • Work in a safe manner as per relevant health and safety legislation and NFCHC policies.
  • Ability to work extended hours, weekends, and other hours as requested by leadership.
  • Any other reasonable duties as required.

Education and Employment Requirements:

  • Current registration and in good standing with the College of Nurses of Ontario.
  • Post-secondary academic credentials in Nursing from a recognized institution.
  • Knowledge of child health, development, and community-based healthcare models.
  • Excellent communication and interpersonal skills to work effectively with children, families, and healthcare professionals.
  • Commitment to fostering an inclusive and culturally competent care environment.
  • Maintaining an understanding of the health issues of the community to provide appropriate services.
  • Proven ability to provide compassionate, respectful, non-judgmental and culturally competent care to the disenfranchised and primary populations.
  • Previous leadership experience in pediatric healthcare or community health settings an asset.
  • Demonstrated communication, time management, problem solving and organizational skills.
  • Previous experience establishing collaborative working relationships with a multi-disciplinary team, clients and community service providers.
  • Ability to problem-solve and apply sound judgment.
  • Proficiency in the use of computer software/electronic medical records.
