Pay Scale: $38.145/hr to $44.882/hr

Job Summary
The West End Integrated Falls Prevention Program aims to address falls prevention strategies and treatment, improve mobility, and enhance overall quality of life for seniors living in the community and their caregivers. The Registered Nurse will work with a multi-disciplinary health care team in the provision of individual client falls assessment, intervention, education and outreach based on best practices.
Job Specific Responsibilities
- Using a client centered approach, the Registered Nurse Provides individual assessment, education, and outreach to seniors.
- Within the scope of a registered nurse including appropriate assessments and counseling, screening, referral, education, and follow-up and other clinical activities consistent with the College of Nurses of Ontario standards of nursing practice and in collaboration with the WEIFPP team.
- Keeps complete, accurate and timely records of client care.
- Works autonomously in community setting through outreach and home visits and will assess, manage and document clinical risk.
- Acts as a health resource and provides appropriate support and consultation to staff other health care team staff.
- Participates in the preparation of new materials based on best practices for client education.
- Participates in regular chart reviews and case conferences.
- Provides health promotion activities as appropriate to the position.
- In consultation with the Program Manager and health care team, develop policies, procedures and protocols for client management and for quality care.
- Participates actively in information sharing within the program, team and other partner agencies.
- Assists in the collection and analysis of relevant data for administration, planning and evaluation.
- Works with internal and external service providers as well as relevant community groups.
- Participates in the development of primary care and health education programs based on client and community identified health needs.
- Participates in community outreach activities related to falls prevention.
- Performs other tasks related to the Integrated Falls Program as assigned by Supervisor.

- Maintaining competence, and a professional license to practice.
- Supporting the Centre’s student and volunteer placement programs.
- Promoting awareness of and participation in Centre activities.
- Contributing to the Centre’s work by participating in meetings and committees.
- Working during both regular and extended hours of operation in locations identified by the Centre.
- Contributing to the Centre’s practices of hiring, orienting, and training of staff.
- Contributing to the Centre’s efforts to secure and maximize resources for current and new programs, services, and activities.
- Participating in the Centre’s efforts to enhance its capacity through staff development.
- Working in a manner that supports the Centre’s Occupational Health and Safety policies and procedure
Qualifications
- Baccalaureate Degree in Nursing.
- Current (and in good standing) Registration with the College of Nurses of Ontario.
- Minimum of 2 years of nursing practice in a community setting
- Professional nursing experience working with seniors, low-income families and multicultural communities.
- Self-motivated, organized, and able to work independently while collaborating effectively with a team
- Demonstrated ability to work in a multi-disciplinary team environment.
- English essential; Bilingualism and other languages an asset.
- Administrative Skills: completing reports and maintaining client records; working in a computerized (Windows) environment.
- Valid driver’s license and current, adequate vehicle insurance, use of a vehicle.

Founded by community members in 1979, we are one of about 60 non-profit community health centers across Ontario. We are run by a volunteer Board of Directors comprised of 12 community members elected at our annual meeting.
Growing With Our Community:
In 1979 we opened our doors with 4 staff. Today, we have over 347 full and part time employees serving our local community and over 400 volunteers.
