Position Summary:
The Primary Care Nurse will work to full scope of practice to support multiple clinical services delivered in Grande Prairie Primary Care Network’s (PCN) central facility. These services include but are not limited to persistent pain services, women’s health clinic, and preventative care for unattached patients.
The Primary Care Nurse will assess patient needs, communicate and support treatment recommendations, provide health education, and, under the direction of a Nurse Practitioner or Physician, provide procedural care and arrange for tests as appropriate. The nurse will collaborate with the entire PCN multidisciplinary team to manage patients and coordinate appropriate referrals to community care or specialists when required.
This is a 0.8 FTE (30hrs per week) permanent position and may include occasional evening or weekend shifts.

What you will do:
- Work within their scope of practice as part of the clinic team to provide support to primary care providers and patients.
- Coordinate sharing of patient medical information with clinic team members
- Perform a variety of nursing duties to provide consistent high-quality care to meet patient needs.
- Assess patient conditions, needs, and goals in collaboration with the patient, primary care provider and multidisciplinary team.
- Provide relevant patient education to prevent and manage clinical conditions.
- Ensure the maintenance of accurate records in line with practice policy and organizational standards.
- Assist patients in navigating the health system and provide direction in accessing local and regional health programs.
- Collaborate with the multidisciplinary team to determine health services required based on presenting concerns and test results.
- Support patients by facilitating appropriate referrals to community resources and programs.
- Participate in the evaluation of PCN services.
- Assist in planning and monitoring activities.
- Assist in the development of policies and protocols for clinic activities.
- Assist in setting up health promotion programs relevant to the needs of the practice population.
- Assist in developing and implementing patient self-management supports.
- Assist in the implementation of strategies for improving the quality and provision of care for the practice population.
REQUIRED QUALIFICATIONS:
What you need to succeeed:
- Successful completion of an accredited Licenced Practical Nurse diploma program or a bachelor’s degree in nursing
- Three years or more of related nursing experience
- Current CLPNA Registration or CRNA practice permit in good standing and current BLS-HCP
- Clinical experience in primary care nursing or primary care practice settings an asset
- Training as Behavioural Health Consultant (BHC) considered an asset
- Clinical skills in primary care nursing
- Knowledge of nursing processes and practices, including intervention strategies and therapies
- Strong oral, written and analytical skills; self-directed and accountable and high level of perceptiveness
- Ability to maintain patient privacy and confidentiality
- Attention to details and problem-solving and critical thinking skills
- Able to work independently and demonstrates ability to be proactive in completing duties
- Strong organizational and time management skills, including the ability to handle multiple assignments and tasks while meeting deadlines
- Ability to remain calm in difficult situations and deal effectively with those who are in distress
- Ability to maintain effective working relations with employees and patients
- Ability to maintain accuracy while multi-tasking or working under pressure
- Ability to collaborate with other multi-disciplinary team members
