In accordance with Article 9.05, where the previous incumbent returns to this position within the thirty (30) day trial period this position shall be deemed not vacant and the posting will be cancelled. Should the vacancy be cancelled, applicants to this posting shall be notified.

PRIMARY FUNCTION:

Reporting to the Program Manager – KGH-MHAP, the RAI Coordinator will primarily coordinate with nursing staff regarding the collection of information related to the Resident Assessment Instrument – Mental Health (RAI-MH), and in some cases complete the needed documentation by working collaboratively with the KGH-MHAP Clinical Team.

Within this role the employee is accountable for contributing to the delivery of the Kingston Health Sciences Annual Strategic Plan as well as the Strategic Plan for Mental Health & Addiction Care. As an employee, one must demonstrate an awareness of and be responsible for actively promoting and supporting patient and family centered engagement and care in all we do**.**

PRINCIPLE RESPONSIBILITIES:

  • Searches the electronic patient chart in order to monitor and coordinate RAI-MH assessment with nurses in the KGH-MHAP, and will comlete the assessments if needed. Performs data entry into the Health Information System (Lumeo) as required by legislation.
  • Facilitates effective use of the data to support recovery focused client care and client/family collaboration in treatment and discharge planning, by producing reports and bringing forward trends to Manager or delegate.
  • Consults and collaborates with clinical teams and site experts to ensure complete and accurate entry of data and encourages and facilitate the clinical utilization of RAI-MH data.
  • Communicates effectively with Program Leadership and/or the Lumeo Regional Team to identify any concerns regarding data submission. Ensures that timelines are met, and therefore enabling data to be checked for errors and corrections made prior to submission.
  • Assists the Program Operational Director/Program Manager and staff to plan, coordinate and evaluate quality improvement initiatives as they relate to RAI data.
  • Provides education/learning associated to changes in the system.
  • Workplace Health & Safety/Workplace Conduct:
  • Adheres to worker responsibilities as set out in the Occupational Health & Safety Act, hospital safety policies, and dept. /unit established procedures at all times.

  • Takes corrective action to address a hazard where possible, otherwise reports the unresolved hazard, dangerous circumstances, unsafe situation to self, other co-workers or the community, to their supervisor.

  • Advises supervisor if they incur a workplace injury or illness; actively participate in identifying and resolving the underlying cause(s), and participates in early and safe return to work as per Workplace Safety & Insurance Act.

  • Understands and is familiar with all pertinent KHSC policies and procedures including those relating to workplace conduct. Complies with the KHSC Commitment to uphold the Workplace Conduct and Reporting of Inappropriate Conduct policy and behaves in a manner that is consistent with the guiding principles and expectations.

QUALIFICATIONS:

  • Graduate of a diploma Registered Practical Nurse program from a Community College.

  • A member in good standing with the College of Nurses of Ontario or eligible for and seeking registration with the College of Nurses of Ontario.

  • 1-year recent experience working in Mental Health & Addiction/Psychiatric Nursing within the last 5 years.

  • Demonstrated ability to utilize the existing Health Information System (Lumeo) to track, monitor and complete RAI-MH submissions.

  • Demonstrated ability to utilize computer software program such as: Microsoft Office, Email, and patient care systems used within the organization.

  • Experience with performance measurement and quality improvement methodologies preferred.

  • Demonstrated ability to effectively assess patients in various mental health patient populations.

  • Demonstrated ability to effectively contribute to the development and evaluation of treatment plans for patients in various mental health patient populations.

  • Demonstrated understanding of the RAI-MH system.

  • Demonstrated ability to support change initiatives and best practice alignment.

  • Demonstrated ability to understand and advocate the RAI-MH uses and benefits in relation to clinical practice.

  • Knowledge of quality improvement and client safety frameworks, tools and principles and relevant legislation, regulations and their applications.

  • Demonstrated ability to effectively organize, and prioritize multiple competing demands in a busy office environment and ability to plan, coordinate, and implement projects and short and long range goals.

  • Excellent verbal and written communication skills.

  • Ability to link with regional peers to identify best practice and implement into daily clinical practice.

  • Demonstrated trouble shooting skills and ability to problem solve issues that may arise in carrying out the duties of this position.

  • Demonstrated understanding and commitment to health and safety practices, procedures and applicable legislation.

  • Demonstrated professional work ethics and behaviour in interaction with interprofessional teams internal and external stakeholders.

  • Demonstrated effectiveness working with minimal supervision with proven reliability and trustworthiness while working toward achieving required outcomes.

  • Experience completing and applying RAI-MH assessment data and information into daily clinical practice required.

  • Proven ability to attend work regularly.

  • Satisfactory criminal reference check and vulnerable sector search required.

PHYSICAL REQUIREMENTS:

The applicant must be able to meet the physical demands of this position.
