Acclaim Health announces the availability of one (1) Full-Time Care Manager, Nursing (RN) position with the Home Care department working in the Oakville Office. The incumbent will work Monday to Friday from 8:30am - 5:00pm.
PRIMARY FUNCTIONS:
- In collaboration with Human Resources, manages the recruitment of new nursing staff
- Effectively manages the performance of direct reports through direct monitoring and regular performance reviews
- Provides input into the development of the nursing education plan
- Ensures that direct reports understand and are aligned to their key accountabilities and perform to the expected level
- Leads and facilitates Nursing Team meetings
- Oversees daily and monthly scheduling of nurses
- Provides internal customer service excellence through leading by example utilizing coaching and mentoring to support employees in providing excellence in patient care
- Provides evening and weekend on-call service for Health Services on a rotational basis
- Oversees the flu vaccine program
- Participates and collaborates with a multidisciplinary care team including staff, patients and caregivers in supporting the patient’s needs according to the patient’s care plan
- Works in collaboration with the Service Coordination Team to ensure patient-focused scheduling
- Provides clinical consultation using resources such as the CNO and best practice guidelines of the RNAO
- Ensures compliance with funder contracts including ensuring that nurses complete all relevant funder reporting
- Responds to patient enquiries, concerns, changing patient needs and ensures ongoing patient satisfaction and patient safety
QUALIFICATIONS & EDUCATION:
- Baccalaureate Degree, Nursing or equivalent.
- Valid Ontario driver’s license, use of vehicle and insurance per Acclaim Health policy.
EXPERIENCE & SKILLS:
- Minimum 3-5 years as a Registered Nurse. Supervisory or leadership experience an asset.
- Demonstrated ability to coach, mentor and lead staff
- Effective verbal and written communication skills.
- Demonstrated assessment, problem-solving and negotiating skills.
- Strong planning, coordinating and organizing skills
- Demonstrated ability to work independently and collaboratively as a team member
- Demonstrated knowledge and skills in quality/risk management
- Strong customer service skills
- Proven ability to multi-task and meet deadlines
- Proficient in Microsoft Office, HPG and AlayaCare
- Knowledge of community resources
