Cassellholme, a not for profit, charitable organization, operates a 240 bed Long-Term Care facility, and a Community Support Services program in the district of East Nipissing. Cassellholme is located in the City of North Bay which is situated within 3 ½ hours of both Toronto and Ottawa. Nestled between beautiful Lake Nipissing and Trout Lake, the district offers both urban and country living, with multiple recreational opportunities.
Cassellholme is seeking a NURSE PRACTITIONER to work on a permanent, full-time basis as an inter-professional health care team member.
What can I expect to do in this role?
In this role, you will use your specialized knowledge and skills to -
- Help increase access to, and quality of, primary health care by providing proactive screening and assessment, timely specialist referrals, follow-up care, ongoing chronic disease management, and end of life care;
- Research and implement evidence-based practices;
- Collaborate with the Director of Care in the provision and advancement of nursing services in the Home;
- Increase knowledge capacity of staff and programming to meet the needs of residents;
- Improve care coordination and collaboration with health care providers across the care continuum;
- Practice in accordance with College of Nurses of Ontario Practice Standards and Ministry expectations
How do I qualify? Mandatory Requirements You are registered as a Registered Nurse Extended Class (Nurse Practitioner) in good standing with the College of Nurses of Ontario
- You have a Nurse Practitioner specialty certificate in primary health care or adult care
- You have a current certificate of competence as a Registered Nurse with Extended Class privileges with the College of Nurses of Ontario;
- Experience as a primary health care nurse practitioner, gerontological nursing experience is an asset;
- You have a satisfactory criminal police information check with vulnerable sector screening
