About the role

Wahkomatowin Seniors Wellness Program focuses on providing opportunities for older adults (55+) to connect to others in their age range, to community, and to resources they may require. We provide fun interactive programing with learning sessions for identified interests by the participants. Our program is part of the multi-partnered Seniors Coordinated Outreach.

Seniors Coordinated Outreach is a collaboration comprised of organizations across Edmonton that have Outreach and Support workers who can help connect Older Adults to relevant services and programs in our city.

Responsibilities

  • Provide Outreach support/resources to Seniors accessing services through 211.
  • Assist seniors to connect to partner agencies, services, and supports.
  • Develop community partnerships and promote senior programs.
  • Keep contact notes and vital information on Outreach and home or community visits.
  • Attend all meetings/supervisions and partner meetings.
  • Submit monthly time sheet and other paperwork.
  • Having cultural/traditional knowledge is an asset.
  • Arrange and attend home visits with participants.
  • Facilitate conversations between family members and professionals to support the older adult.
  • Advocate for older adults when needed.
  • Engage older adult’s families for support when needed.

Qualification Requirements

  • Degree or diploma in Human Services field
  • Experience with home visitation and resource/referral
  • Knowledge of Senior concerns and knowledgeable regarding resources for this age demographic
  • Trauma informed training an asset
  • Current (within 6 months) Criminal Record
  • Current (within 6 months) Child and Family Intervention Record
  • Must have own vehicle for transportation with valid (without issue) current driver’s license and driver’s abstract.
  • Experience working with Indigenous people an asset.
  • Knowledge of indigenous languages an asset.

Salary and Benefits

  • Salary plus monthly auto allowance.
  • Paid sick and wellness days after 3 months’ probation (pro-rated)

  • Working approximately 34.5 hours per week
