Family Health Teams (FHTs) use a patient-centered approach to primary health care that brings together doctors, nurses, nurse practitioners, registered dietitians, pharmacists, mental health clinicians and other health care professionals and support staff to co-ordinate the highest possible quality of care for patients – when they need it, as close to home as possible. FHTs are a key component of Ontario’s healthcare plan to reduce wait times, improve access to primary care, and keep Ontarians healthy.
Overview Reporting to the Primary Care Manager(s) at the Peterborough Family Health Team (PFHT), the Registered Practical Nurse (RPN) is an important member of a multidisciplinary team. The RPN provides nursing care and clinical support within their full scope of practice as defined by the College of Nurses of Ontario (CNO) Standards of Practice. Working in collaboration with physicians, Registered Nurses, and other team members, the RPN supports the delivery of high-quality, person-centered care across a variety of clinical and preventative services. This is a permanent, full-time (1.0 FTE) position based at the Chemong Medical Centre in Bridgenorth, Ontario.
Responsibilities
- Conduct patient interviews to gather information about the presenting health concerns
- Perform focused assessments based on patient need, using appropriate tools and techniques
- Document relevant health history, medications (including OTC and alternative), allergies, and intolerances in the EMR
- Maintain accurate and up-to-date medication records, reviewing regularly with patients
- Apply clinical reasoning to identify and communicate potential health concerns
- Utilize best practice guidelines for monitoring and supporting patients’ physical and emotional health
- Incorporate lab results, diagnostic tests, and specialist reports to inform care planning
- Provide in-person and telephone-based triage and support, following established protocols
- Assist with routine procedures, including immunization administration, wound care, and vital signs monitoring
- Support chronic disease management initiatives in collaboration with other clinical team members
- Participate in preventative health activities, such as flu clinics
- Maintain clinical documentation in accordance with organizational and professional standards
- Other duties as assigned to support the effective operation of the clinic and PFHT programs and services
- Participate in regular PFHT meetings, trainings, and programs as required
Position Requirements
- Ability to perform all clinical and technical tasks within the RPN scope of practice
- Strong interpersonal and communication skills with sound clinical judgment
- Ability to work independently and collaboratively within a busy, multidisciplinary environment
- Proficient in the use of Electronic Medical Records and other relevant digital tools
- Understanding of how social determinants of health impact individual and community health
- Demonstrated commitment to anti-racism, anti-oppression, equity, and inclusion principles
- Demonstrated problem-solving skills and adaptability in a fast-paced environment
- Familiarity with primary care and how it intersects with other parts of the healthcare system
- Willingness to work in full scope of practice, performing delegated acts through medical directives
- A valid driver’s license and reliable vehicle (travel to various clinic locations may be required)
- Clear vulnerable sector police check
- Willingness to participate in data collection for quality improvement and program development
- Ability to lift up to 20 lbs
- Flexibility to adapt to changing hours and clinical demands
Qualifications
- Diploma in Practical Nursing
- Current registration with the CNO and active knowledge of CNO Standards of Practice and Best Practice Guidelines
- Minimum 3 years’ clinical experience
- Excellent documentation and time management skills
- Experience in immunization administration
- Well-women’s and well-baby assessment skills an asset
- Experience in primary care or community health settings preferred
- Member of WeRPN preferred

As one of the largest Family Health Teams in Ontario, the organization works with over 100 primary care physicians providing comprehensive care to their patients. Collectively, we have over 100 inter-professional healthcare providers, comprised of: mental health clinicians, nurse practitioners, pharmacists, a physician assistant, registered dietitians, registered nurses and registered practical nurses.
In addition to one-on-one care, PFHT administers many programs, services, and workshops as a means to redirect patients from the providers practice to a more focused area of care.
