1 ‑ Full Time ‑ Nurse Practitioner

Medical Transitions Clinic

The Nurse Practitioner, Medical Transitions Clinic, under the direction of the Manager, Medical Transitions Clinic,

Internal Medicine Clinics, Complex Care Diabetes Centre, Regional Bariatric Care Centre, Paediatric Healthy Living

& Telemedicine will be responsible for the direct patient care of patients who are referred to the program to help

provide transition of care back to the community. One of the primary responsibilities will be supporting the Outpatient

Parenteral Antimicrobial Therapy (OPAT) clinic.

EMPLOYMENT REQUIREMENTS:

Education/Experience:

* Unencumbered registration as a Registered Nurse in the Extended Class specialty certificate Primary Health Care

with the College of Nurses of Ontario

* Master's degree in Nursing

* Active Professional Liability Coverage required

* Membership with RNAO or NPAO is preferred

* Experience in the development of clinical guidelines and protocols, promoting the use of research and evidence

based practice to guide patient centered care is required.

* Current certification in Basic Cardiac Life Support

Skills/Abilities:

* 2 years' experience as a Nurse Practitioner, preferably experience with acute care, wound care, intravenous

antibiotics

* A commitment to developing equity‑based, evidence‑informed approaches for delivering care services.

* Ability to work well under pressure and use good judgment in assessing difficult situations.

* Ability to work independently and as part of a team.

* Aptitude for proactive problem solving, using strong critical thinking, and negotiating skills.

* A demonstrated ability to develop, deliver and evaluate training and coaching efforts.

* An ability to develop relationships based on trust, compassion, and empathy.

* Strong active listening and interpersonal communication skills for one‑on‑one coaching.

* A demonstrated focus on patient and family centred care.

* Supports the advancement of equity, inclusion, and diversity.

* Proficiency in technologies (e.g., webinars, apps) as well as knowledge of Microsoft Outlook, Word, Teams,

PowerPoint, and Excel.

Assets:

* Regular and sustained attendance as per hospital standard

* Knowledge and adherence to the hospital's Code of Conduct

Salary Range: $65.210 to 70.750
