POSITION SUMMARY:
Reporting to the Senior Director, Client and Family Integrated Care, Transitions and Community Partnerships, the Manager, Client and Family Integrated Care and Community Partnerships works within the program’s philosophy and framework to advance client and family-centred care (CFCC) and partnership at all levels of hospital decision-making. They are a lead consultant on the principles and best practices that foster staff, client and family partnership across governance and policy-making, research, education, program and service design, and clinical care delivery.
The Manager is responsible for leading the Client and Family Integrated Care team, including Holland Bloorview’s award-winning Family Leadership Program, which advances client and family-centred care initiatives across the organization. This role provides strategic guidance and expert consultation across the hospital and the broader health system, playing a pivotal role in enabling cross-program collaboration that nurtures an authentic, quality-focused, and equity-driven culture centered on client and family care and exceptional client and family experiences. Key partners include clients and families (such as Youth and Family Leaders), students, clinicians, researchers, staff, hospital leaders, and diverse community stakeholders.
KEY RESPONSIBILITIES:
Program and Staff Management – Client and Family Integrated Care
- Responsible for operational oversight of the Client and Family Integrated Care team, including the Family Leadership Program.
- Provides direct leadership, supervision, and support to a team of staff, ensuring effective performance evaluation, professional development, and alignment with organizational goals.
- Responsible for effectively managing assigned people/human resources including employee engagement, coaching, recruitment, orientation, retention, performance development, and evaluation.
- Provides financial oversight for program areas, including budget development, monitoring, and reporting.
- Leads the preparation of grant proposals and accountability reports, ensuring alignment with organizational goals and compliance requirements.
- Leads consultation on organizational client and family engagement initiatives, ensuring a client- and family-centered approach is embedded in all aspects of service delivery and program development.
- Fosters authentic engagement between clients, families and staff using leading principles and best practices in client and family centered care.
- Leads and supports initiatives that engage equity-deserving populations, embedding principles of Inclusion, Diversity, Equity, Accessibility, and Anti-Racism (IDEAA) to ensure culturally responsive, inclusive, and anti-oppressive client and family-centered care and partnerships.
- Partners with stakeholders to innovate new client and family centered care education and training and consult on a range of staff/family co-design initiatives at the governance, program and service, and direct care levels.
- Builds and fosters internal organizational partnerships and external community partnerships to strengthen and enhance the delivery of Client and Family Integrated Care programs and services.
- Represents the Client and Family Integrated Care team at committees and task forces across the organization.
- Participates in Manager-on-Call rotations and coverage.
Client and Family-Centered Care Education, Training & Evaluation
- Partners with staff, educators, clients and families to co-design and deliver simulation-based client and family-centered care training, both across the hospital and health system.
- Consults with staff on client and family-centered care initiatives across the organization and in the community.
- Develops and applies robust evaluation methods and indicators to measure the success of client and family-centered care and partnership across the organization.
- Uses evaluation and data around client and family experience to make recommendations for improvement.
- Contributes to academic papers and knowledge production highlighting program initiatives.
Health System Outreach and Partnership Building
- Collaborates with stakeholders to establish and sustain external partnerships that advance the Client and Family Integrated Care Program.
- Participates in relevant communities of practice, forums and strategic tables across the health-system.
- Delivers external presentations to promote the hospital’s model for client and family-centred care and client and family partnership.
- Consults external organizations on client and family-centered care principles and concepts and leading models for client and family partnership.
- Advances the system-wide profile, scale and impact of the Family Leadership Program.
QUALIFICATIONS
- Master’s degree in healthcare administration, public health, social work, or a related field. Project management or healthcare leadership certification is an asset
- Minimum of three to five years of experience in a manager-level or equivalent leadership role within healthcare, community, or related sectors is required
- Lived experience and/or formal training in client and family-centred care and partnership approaches (e.g., participation in certificate programs, courses, or conferences) is preferred
- Lived experience with pediatric rehabilitation and disability and/or caregiving is considered an asset
- Well-developed understanding and demonstrated ability to apply health equity, anti-oppression, and population health frameworks, including how social determinants of health impact client and family experience
- Excellent project management skills including the ability to plan and collaborate with teams across the hospital
- Proven ability to build and sustain collaborative relationships across healthcare, community, and broader system settings to support shared goals and integrated approaches
- Proven ability to collect, analyze, and apply data to evaluate programs and initiatives, inform decision-making, and drive continuous improvement
- Visionary and values-driven leader with a demonstrated commitment to advancing client and family partnerships
- Highly developed interpersonal and problem-solving skills
- Ability to develop and nurture trusting relationships with individuals and teams
