About the role

Family Practitioner – Sooke Outreach Services (Western Communities PCN) 100-2155

A part-time (0.2 FTE) Family Practitioner (FP) role is available to deliver outreach services in Sooke, B.C. at the Sooke Food Bank and Sooke Shelter. Services will be provided to adults and children who are impacted by poverty, colonization, stigma, and homelessness. Providing accessible primary episodic care services that not only address immediate medical needs but also actively work towards decreasing barriers and social determinants of health in accessing care.

The FP will be supported by a primary care RN, an MHSU RN and a MOA and provide both OAT and primary care services to a broad range of clients.

A waterfront community near Victoria, British Columbia, the Westshore features a rich heritage and a unique connection to nature. A short drive into the downtown Victoria core and an overall lower cost of living, the Westshore is a great choice to live and work. Cycle or walk the famous Galloping Goose Regional Trail, which winds through 55 picturesque kilometers of the Westshore, or discover the surrounding lakes (Langford Lake, Glen Lake and Florence Lake) and hike Mount Finlayson, Mount Wells and Goldstream Provincial Park.

One-two days a week, 8 hours/week routinely providing services on Thursdays. On-call expectation: Inbox management only with potential of on call one day/week (dependent on provider’s working hours for the week).
