Job Description:
The Mental Health and Addiction Day Program serves as a step down level of care for those being discharged from inpatient mental health and as a less restrictive alternative to inpatient admission for individuals presenting to the Emergency Department or referred by the community. The Day Program includes intensive group therapy, brief transitional case management, mindfulness meditation, and individual and family counselling and support as required. The department consists of an inter-professional treatment team and therapy is based on a solution focused short term model of care.
As the Case Manager, you will provide support, direction, and guidance to individuals experiencing serious mental illness, who live within the community; to help maintain wellness and improve quality of life through routine and structure. This support includes but is not limited to: life skills training, health teaching, individual case management, group facilitation, crisis management, assistance with activities of daily living, monitoring of medication compliance and psychiatric appointments, liaising with families and community partners, and assistance with volunteering and employment goals.
As a qualified candidate you will have the following job requirements:

Secondary School Diploma or Equivalent
-
Three (3) year post-secondary diploma in Nursing, Social Services Worker, or other health related field
-
Regulated health professional preferred
-
Current First Aid/CPR/NVCI training
-
Five (5) years of experience working with the seriously mentally ill population
-
Five (5) years of experience using assessment skills, problem-solving skills, confidentiality, independent judgement, autonomous worker, communication and time management skills, crisis intervention skills, confident self-starter, computer literate, group facilitation, and leadership
-
Two (2) years of experience demonstrating the ability to work independently as well as in a team setting, good interpersonal, time management, organizational skills, computer literate, flexible and compassionate.

Exceptional verbal and written communication skills

Proficient at documenting client/patient information using various computer programs
About Royal Victoria Regional Health Centre
Royal Victoria Regional Health Centre, located in Barrie, Ontario, provides safe, high-quality care for residents across a large geographical region including Simcoe County and the District of Muskoka. The area is home to more than 450,000 residents, with half of RVH’s patients living outside the City of Barrie.
As the largest health centre in North Simcoe Muskoka, RVH provides highly specialized services, and technology not found anywhere else in the region. This includes the only interventional radiology suites for minimally invasive surgeries, vascular services, advanced stroke care, as well as dedicated trauma rooms in its large Emergency department. RVH is also home to the Hudson Regional Cancer Centre, Simcoe Muskoka Regional Heart Program, Simcoe Muskoka Regional Child and Youth Program and the RVH Regional Renal Program meaning distance is no longer a factor for residents receiving lifesaving treatment.
