An exciting opportunity exists for the right individuals to work as Assisted Independent Living Case Managers. The successful candidates will be joining a progressive non-profit organization with growing opportunities.
The Assisted Independent Living (AIL) Program provides accommodations and supports to adults with acquired brain injury, mental health diagnoses, as well as physical, developmental, and medical healthcare needs to provide them with resources to live healthy and fulfilling lives at home and in the community.

Some benefits have eligibility requirements of surpassing three (3) months and securing a regular position of 20hrs/week or more. The benefits and criteria may change from time to time at the discretion of Strive.
Strive ensures the health and safety of all employees by acting with an abundance of caution and adhering to guidelines from the Ministry of Health, mandating regular safety training, and providing employees with access to personal protective equipment.
Positions available:
- Monday - Friday 8:30am-5:00pm (Full-time, Permanent)
Work collaboratively with the department Director and Program Manager to maintain the program in a fun and flexible environment. Coordinate and manage a caseload of assigned responsibilities. Act as a resource to individuals served, their respective family members or representatives and the care providers. Perform a variety of professional activities in support of the Program including but not limited to overseeing contracted community support homes, connecting with contracted care providers in the community to oversee service provision, coordinating OT, PT and other health care-related services to support the individual in service, collaborating with stakeholders, reporting to Funders regarding critical planning and incidents, assisting new participants with transitioning into the program, overseeing individualized client-centered service plans and ensuring all standards are upheld.
Job Qualifications include:
- Degree in Psychology, Social Sciences, Human Services, PT, OT, Behavioural Interventionist/Counsellor, Criminology, Social Work or equivalent
Job Requirements include:
- Criminal Record Check
- TB Test
- First Aid and CPR Level C
- Valid Class 5 BCDL and reliable vehicle to use for work purposes
- Mandt 2-Day (high-risk clients) - Training provided after hiring
Additional Requirements:
