Under long-term contract with Fraser Health, Team Y Foundation is looking for great candidates to become a part of our compassionate patient support team. Our care team support each other to assist the client with disabilities.

Summary of job duties:

You will work at the private home of the client, providing comfort and support, executing the care plan and assisting with daily living needs.

Responsibilities:

  • Must be willing to take the initiative and do what it takes to ensure the client is living the best quality of life possible;
  • Follow the care plan, observe and report any substantial findings and/or changes in patient/resident/client’s behavior to the appropriate member of the care team;
  • Operate mobility equipment (patient lift, wheelchair, hospital bed, etc) to assist with ambulation, positioning and transferring;
  • Maintain and operate respiratory equipment and process with the focus on details and client needs;
  • Help client with daily routine, including exercises, toileting, grooming, getting dressed and other activities as required;
  • Administer PEG tube feeding and follow schedule for meals and medication;
  • Prepare nutritious meals and carry out light household duties with high quality and pride;
  • Able to work as part of a team and to handle multiple tasks safely and effectively;
  • Adhere to professional standards, follow policies and procedures and abide by relevant laws and regulations;
  • High tolerance for working under pressure and handling critical situations with a cool head;
  • Other personal support duties as assigned.

Requirements:

  • Must be reliable, punctual, kind, caring, and truly enthusiastic about their job;
  • Good communication skills, including the ability to communicate sensitive information with empathy;
  • Immunizations (including COVID vaccines) and current TB test completed within 1 year;
  • Current Criminal Record Check (Vulnerable Sector) completed within 1 year;
  • Are legally able to work in Canada.

Qualifications:

  • care, personal support, rehabilitation and/or respite care.
  • Special consideration will be made for applicants with experience working as a Registered Nurse internationally, Nursing Assistant, License Practical Nurse, and Personal Support Worker (PSW).
  • Ability to speak English well.
  • Basic computer skills.
