JOB TITLE: Rapid Re-Housing and Diversion Worker (Temporary 9-weeks, Unionized)
POSITION LEVEL: Experienced; non-manager
POSITION TYPE: Contract, 9 weeks.
Expected start date: July 7, 2025.
HOURS: 35 hours/week; Monday to Friday 8:30am to 4:30pm; with some flexibility, including 1 afternoon shift/week on Tuesdays from 12pm to 8pm
JOB RESPONSIBILITIES AND DUTIES:
- Respond to shelter referrals and daily client work using a Housing First and harm reduction approach
- Assess youth in hopes of avoiding emergency shelter admission; divert youth to safe and appropriate alternative housing options, if feasible and available;
- Conduct a needs assessment, provide information and referral services, and use mediation skills with youth, family members and/or other support personnel to implement strategies that prevent future loss of housing;
- Work collaboratively with the case management team, clinical team, supportive housing staff and community partners to help facilitate diversion
- Maintain client files, housing placements, clinical records, and other documents as pertinent to youth diversion
- Individuals must have the ability to function in high-pressure situations and respond quickly to individuals in crisis
- Conduct risk and needs assessments and safety planning to reduce potential threats
- Make appropriate supported referrals to housing, health and community services and provide advocacy support services with individuals
- Follow up and coordinate with other service providers to ensure clients are referred to appropriate supports
- Ensure regular housing related contact with the participant for a minimum of one year
- Apply documented, structured case planning with identified goals to ensure participant interactions have objectives and are outcome oriented ; connect with case management team based on intensity of needs
- Broker and advocate with other system elements, accompany clients to appointments, provide proactive crisis planning, organize meaningful daily activity opportunities, liaise with Ontario Works and Ontario Disability Support Program
- Prepare and plan for “homelessness proofing” and re-housing. Includes recognizing and averting indicators that may de-stabilize a clients housing and focus on what worked when re-housing a client
- Apply a collaborative approach to case planning and record keeping. Collect and record demographic information and maintain detailed case notes, case management plans and contact lists of involved supports and community services using the integrated data management program
- Maintain a project budget
- Provide public presentations to community service providers
- Provide after-hours support as scheduled
- Participate in Wyndham House team meetings
- Other duties as assigned

JOB SKILLS AND QUALIFICATIONS:
- Education: Completed post-secondary education in social services or related field
- 2+ years work experience in addictions, mental health and case management experience
- 2+ years experience delivering community-based services
- A working knowledge of Residential Tenancies Act
- Experience in mobilizing other community resources in support of the youth’s retention of existing housing or acquisition of new housing
- Considerable experience addressing the needs of individuals experiencing homelessness or those at risk of homelessness including developing and implementing successful case management plans
- Advanced skills to enhance participant motivation in order to make and follow through on decisions related to treatment, stability and wellness
- Experience in assisting persons living with addictions, mental health, psycho/social/physical, educational, emotional and/or environmental concerns in a social service setting
- Understanding of community agencies and a cross section of service providers
- Knowledge of services to support this population and an ability to provide participant advocacy and proactive participant planning with other system elements
- Experience with ongoing evaluation of complex case management or treatment plans, engaging with the participant and other resources in adjusting the plan, as appropriate
- Experience with conflict resolution practices including "in the moment" problem solving and intervention
- Ability to work in a highly stressful and ever-changing environment
- Must have excellent boundaries
REQUIREMENTS:
- Must have a valid driver’s license, access to a vehicle and $2 million liability insurance
- Satisfactory Police Records Check and Vulnerable Positions Screening
- Two doses of the COVID-19 vaccine
- Standard First Aid and CPR-C
