Cedarhurst Dementia Care Home is a small and secure residence for people living with dementia including Alzheimer's. We offer care for a maximum of 26 residents. Each section in the home has six or seven residents, sharing their own kitchen and dining area, with a dedicated personal support worker. The strong caregiving support within each small group allows residents the comfort of their routines plus the opportunity to enjoy flexibility in their day-to-day activities.
JOB TITLE: Personal Support Worker (PSW)
REPORTS TO: Care Coordinator (CC)
JOB OVERVIEW: The Personal Support Worker (PSW) works under the close supervision of the Care Coordinator (CC), who is a Registered Practical Nurse, to provide personal care to residents to meet their physical, emotional, social, cultural, and spiritual needs while following the Cedarhurst Policies and Procedures manual.
PSWs are collaborative members of the interprofessional care team, assisting in the planning, implementing and ongoing evaluation of individualized resident care plans.
MAJOR JOB RESPONSIBILITIES:
- Assists residents with all Activities of Daily Living (ADL) which included but not limited to the following:
a. Interacting and conversing with residents in a positive manner providing emotional and social support
b. Dressing assistance when needed including individual grooming – shaving, oral care (both routine and special), nail care, foot care
c. Meal assistance
d. Assists any residents with mobility/transfer needs
e. Provides recreational and life enrichment activities appropriate to the resident
f. Bathing – including washing hair as required
g. Regular toileting assistance and continence care and management as per resident needs
- Resident medication administration and monitoring of health status
a. Assists resident with medication administration as per applicable policies
b. Documents food and fluid intake on a daily basis
c. Skin and wound care
d. Collect specimens as directed by CC and/or the DOC
e. Under direction of the CC/DOC and with a transfer of function the PSW may perform and record the vital signs and apply prescription creams and report findings to the CC/DOC
f. Communicates any changes in health status of the resident to the Care Coordinator or Director of Care in a timely basis
g. Completes documentation of all resident care activities accurately on the daily flow sheet according to policy
h. Participates in residence care conferences as required
i. Works with the Care Coordinator and Director of Care to ensure timely communications to resident’s family regarding any health related issues
j. Assists with the care of the body if death occurs, as per policy and procedures
- Operation Requirements
a. Completes shift change report and communicates with on-coming PSWs any resident issues that need immediate attention
b. Maintains a clean and safe environment including making beds, collecting and laundering soiled linens and clothing, emptying bed pans, urinals and commodes, remove unsafe objects from use, and clean resident equipment including walkers and wheelchairs
c. Follows the safety policies as outline by Cedarhurst
d. Keeps Cedarhurst’s leadership aware of any incidents involving resident, family and/or staff, including near misses, injuries, unsafe work habits or working conditions
e. Supports and adheres to Cedarhurst’s infection prevention and control practices
f. Participates in fire drills and other emergency planning to fully understand fire, evacuation and emergency procedures
g. Assists in the maintenance and proper care and storage of all nursing equipment and immediately reports to the CC/DOC any equipment needing repair or replacement.
h. Provides light house duties as required throughout the shift such as sweeping floors, cleaning spills, cleaning and disinfecting bodily fluid spills
i. Does meal preparation
- Administration
a. Attends staff meetings and participates in educational sessions offered
b. Follows the privacy and confidentiality policies and procedures established by Cedarhurst, the retirement homes act and the substitute decision act
c. Participates in Cedarhurst’s quality improvement program as required
d. Performs other tasks that may be assigned from time to time
Qualifications: PSWs have completed the Personal Support Worker’s certificate program through a recognized College or accredited educational institution. Experience in retirement home and/or long-term care settings as well as experience with older adults living with dementia is preferable. An ability to take direction as well as take initiative to safely provide care and programs to meet the individualized resident’s needs is required. The PSW is caring, respectful, demonstrates emotional intelligence attributes, excellent customer service skills, effective communication skills and can build meaningful relationships with others. The PSW will need to work both independently and collaboratively within an interprofessional team maintaining a positive attitude and energy while in the Home.
Job Type: Part-time
Pay: From $23.13 per hour
Expected hours: 15 per week
