Position Summary:

Reporting to the Clinical Manager of Pharmacy Services you will be an integral member of the Pharmacy team and be accountable for the development, implementation and monitoring of initiatives for improvement of drug utilization. The Drug Utilization Evaluation Program provides an ongoing review of drug use at Trillium Health Partners using evidence-based medicine to support recommendations and to develop improvement strategies for drug utilization. You will promote the appropriate, safe and cost effective use of drugs and support the organizational formulary system. You will collaborate with the pharmacy leadership team and provide support and leadership to clinical pharmacists in their clinical and educational activities.

You will also contribute to peer, pharmacy undergraduate, interdisciplinary and pharmacy residency teaching/training and be an active participant in relevant inter-professional hospital committees. On occasion, you will be required to travel between the three hospital sites.

Key Qualifications:

  • Baccalaureate Degree in Pharmacy or Pharm D.
  • Licensed in Ontario to practice pharmacy (Part A of Register)
  • Completion of hospital pharmacy residency or equivalent experience is preferred; equivalent experience is interpreted as three years of experience in a broad range of pharmacy operational, clinical and/or administrative pharmacy practice
  • Minimum 5 years of recent experience in hospital pharmacy practice required
  • Working knowledge of Epic, Microsoft Word, Microsoft Excel, is an asset
  • Active membership in the Canadian Society of Hospital Pharmacists (CSHP) is an asset
  • Strong leadership, interpersonal, problem solving, organizational and communication skills
  • Experience with a formal Drug Information service is an asset
  • Project management experience is an asset Knowledge/Education in Evidence-based Clinical Practice an asset

Responsibilities include but are not limited to :

  • Develops and implements Drug Utilization Evaluation program initiatives to meet established cost saving goals and track the cost-effectiveness of these initiatives.
  • Develops formulary related policies and procedures including parenteral monographs
  • Ability to work with physician groups and lead interdisciplinary teams to affect changes in prescribing
  • Coordinates and participates in administrative and educational aspects of drug formulary maintenance
  • Member of the Pharmaceuticals and Therapeutics (P&T) Committee: coordinates meeting agenda and minutes, present formulary initiatives for committee review, implements approved recommendations.
  • Provides Drug information to THP staff to support patient care
  • Active involvement in teaching pharmacy, medical and nursing staff and/or other health care professionals and students
  • Participates as a member of the Pharmacy Leadership Team
  • Participate in pharmacist clinical and distribution activities as needed to support departmental patient care needs.
  • Participate on relevant hospital committees and professional committees (ie. PSNs, hospital subcommittees, Medication Safety Committee, Monograph Working Group, Pharmacy and Therapeutics Committee, Hazardous Medication Subcommittee, Parenteral Pump Committee etc.)
