Sanctum Care Group Inc. is a Not-For-Profit organization that works directly with populations affected by HIV/AIDS, chronic disease, substance dependence, homelessness, mental health issues, and poverty in Saskatchewan.

PORT coordinates wrap around care between community organizations and pregnant women in complex life situations. PORT provides intensive case management, wraparound supports and services for pre and postnatal women in Prince Albert and surrounding area. Its main function is to provide early and effective interventions in pregnancy that will reduce health complications in mothers and their infants and prevent children from being apprehended at birth.

Wage Range: $38.00 – $43.00 per hour

Main Activities The Team Lead is responsible for administrative tasks, overseeing staff, daily operations, and ensure the highest quality services are provided.

  • HR related functions and duties
  • Education development, training, supervision of staff
  • Ongoing employee evaluation to support care excellence
  • Creating Connections with community agencies and stakeholders in and around Prince Albert
  • Provide case management in a community based setting, assisting with or coordinating;
  • Income Assistance
  • Housing
  • Medical appointments
  • Addictions supports
  • Treatment referrals
  • STBBI Testing
  • Physician referrals
  • Prenatal Care
  • Advocate for clients with Ministry of social services

Qualifications The successful candidates will have the following

  • Exceptional time management, with ability to prioritize and carry out work in a fast-paced environment
  • Strong conflict resolution, organizational and interpersonal skills, critical thinking, exceptional problem-solving ability
  • Willingness to adapt to new concepts, techniques
  • Knowledge in the fields of Child and Family programs, HIV and pregnancy, addictions, harm reduction and trauma informed care
  • Ability to communicate and be assertive while maintaining an effective working relationship with staff and clients
  • Cultural sensitivity to Indigenous issues
  • HR experience, payroll functions are an asset
  • Computer proficiency
  • Ability to work field hours
  • Validate drivers license, access to a vehicle is required
  • Background working in Mental Health and Addictions is an asset
  • BSW or combination of education and work-related experience
